REMARKS

Equipped with a personal fall arrest system or ladder safety system and the system provide protection throughout the entire vertical distance of the fixed ladder for all ladder sections.

The fixed ladder is equipped with a personal fall arrest system or a ladder safety system on one or more sections and landing platforms are provided at maximum intervals of 150 feet.

FIXED LADDERS THAT TERMINATE AT A HATCH

The hatch cover opens with sufficient clearance to provide easy access to or from the fixed ladder.

The hatch cover on the fixed ladder opens at least 70 degrees from the horizontal if the hatch is counter-balanced.

The cage or well is continuous throughout the length of the fixed ladder except for access, egress, and other transfer points.

The cage or well on the fixed ladder is designed, constructed, and maintained to contain employees in the event of a fall.

The cage or well on the fixed ladder is designed, constructed, and maintained to direct employees to a lower landing.

SELF-CLOSING GATES

A self-closing gate or offset passage is provided at the top pf the fixed ladder entrance.

The self-closing gate slides or swings away from the climber on the fixed ladder.

The self-closing gate is equipped with a top rail and midrail or an equivalent intermediate member.

PLATFORMS

The platform of the fixed ladder is at least 24 inches by 30 inches.

FIXED LADDER MORE THAN 20 FEET ABOVE A LOWER LEVEL

The fixed ladder is equipped with a personal fall arrest system, ladder safety system, cage, or well.

The fixed ladder is equipped with a cage or well and sections are offset from adjacent sections.

The fixed ladder is equipped with a cage or well and landing platforms are provided at maximum intervals of 50 feet.

An individual-rung fixed ladder is constructed to prevent an employee's feet from sliding off the ends of the rungs.

CAGES & WELLS

The perpendicular distance from the centerline of the rungs, steps, or grab bars to the nearest permanent object in back of the fixed ladder is at least 7 inches.

A through or side-step fixed ladder side rails extends 42 inches above the top of the access level or landing platform of the ladder.

A through fixed ladder rungs or steps are omitted from the extensions and the side rails are flared to provide at least 24 inches but not more than 30 inches of clearance.

The cage or well on the fixed ladder is designed, constructed, and maintained to permit easy access to, and egress from, the ladder it encloses.

A through fixed ladder, the step-across distance is at least 7 inches and no more than 12 inches to the nearest edge of the structure, building, or equipment accessed from the ladder.

A side-step fixed ladder rungs, steps, and side-rails are continuous in the extension.

A side-step fixed ladder, the step-across distance is at least 15 inches and no more than 20 inches to the access points of the platform edge.

Grab bars extend 42 inches above the access level or landing platform of the fixed ladder.

Grab bars are at least the same size as the rungs of the fixed ladder.

If metal, the fixed ladder is made of corrosion-resistant material or protected against corrosion.

The fixed ladder is capable of supporting the maximum intended load.

The fixed ladder has a pitch of 90 degrees or less.

The rungs, steps, and cleats of the fixed ladder are spaced at least 10 inches and not more than 14 inches apart.

The rungs, steps, and cleats of the fixed ladder have a minimum clear width of 16 inches (measured before installation of a ladder safety system).
