Ladder Inspection

Ladder Inspection

1. OBJECTIVE

1.1. Contractor shall provide all material, transportation, labor, equipment, coordination, planning, and supervision necessary to conduct annual visual inspection on all ladders listed on list provided by Anniston Army Depot (ANAD) and submit a report detailing their findings. All work shall be performed in accordance with the contract specifications and this statement of work.

Contractor shall not proceed with any verbal or written change(s) to the scope of work from sources other than the Contracting Officer, who will provide changes in writing

2. CONTRACTOR’S POINT OF CONTACT

2.1. The Contractor shall provide the Contracting Officer (KO) with a point of contact and a telephone number for the Government to make any arrangements needed. The KO will supply the Contractor with the name and telephone number of his/her designated Contracting Officer Representative (COR) and Contract Specialist. The KO, COR and Contract Specialist shall serve as the Contractor’s point of contact with the government for this task order. The Contractor shall not seek direction or guidance with regard to this task order from anyone else but the designated KO, COR and Contract Specialist.

3. PERIOD OF PERFORMANCE

3.1. The period of performance for this contract will be a base year plus four one-year option periods. The same pricing, terms, and conditions will apply to all twelve month periods of the contract.

5. WORK TASKS

5.1. The contractor shall provide one hundred percent annual inspection on all ladders on the attached inventory list. The annual inspection shall be completed within one year (365 days) of the previous inspection date.

5.2. The contractor shall verify ladder is Occupational Safety and Health Administration (OSHA) compliant, inspect for damage, inspect weld and insure attachment points are secure. The contractor shall inspect safety swing gate for damage and are working properly. The contractor shall inspect fall protection devices (cable and track style and Self retracting lifelines) are working properly. The contractor shall provide, fill out, and attached weather proof inspection tags to each ladder.

5.3. The contractor shall record and report inspection findings/deficiencies and repair recommendation. The contractor shall include photographs in the report. The contractor shall report any unsafe working conditions or potential system failures immediately to ANAD COR and the contracting officer.

5.4. The contractor shall submit inspection reports and photographs within fourteen (14) calendar days following the completion of each inspection.

5.5. The contractor shall ensure that only qualified persons conduct work on this project. The ladder inspector shall be a competent person for fall protection. A competent person is defined by OSHA as a designated person with sufficient experience and/or knowledge to recognize fall hazards, correct unsafe conditions, and who has the ability to shut down the worksite until any hazards are corrected. Certification for all qualified persons shall be provided within five (5) calendar days prior to the start of any work.

5.6. During the performance of the contract, the contractor shall implement fall protection controls based on the type of ladder being inspected and work being performed. The contractor shall be responsible for insuring that all employees conform to the safety guidelines set forth by the Occupational Safety and Health Act (OSHA), American National Standards Institute (ANSI), United States Army Corps of Engineers (USACE) Safety Pamphlet (PAM) 385-1, ANAD Policy and depot safety rules and regulations. The contractor shall use fall protection when required.

5.7. All actions performed shall adhere to all federal, state, local, Depot, and equipment manufacturer’s rules and guidelines. The contractor shall verify all conditions in the field and report discrepancies to the COR for clarification. The contractor shall attend an environmental briefing conducted by the Directorate of Risk Management before start of work. The contractor shall attend required briefing with Defense Logistics Agency (DLA) for entry in their buildings.

6. PROJECT SUBMITTALS

6.1. The contractor shall provide one hard copy and one electronic copy of all submittals to the COR, courtesy copying Contract Specialist for review and approval by the submission deadlines listed below for each submittal. The Government will require fourteen (14) calendar days for review of each submittal or resubmittal furnished by the Contractor for approval. Review and acceptance of submittals by the Government does not relieve the Contractor form the responsibility to meet all specifications and other required contractual standards or regulations.

6.2. The following submittals shall be submitted to the Government:

6.2.1. Repair recommendation and maintenance plan to correct reported deficiencies. (Due fourteen (14) days following completion of inspections)

6.2.2. Inspection report and photographs (Due fourteen (14) calendar days following the completion of each inspection)

6.2.3. Updated ladder spread sheet (Due fourteen (14) calendar days following the completion of each inspection)

6.2.4. Ladder inspector certification. (Due fourteen (14) calendar days after award)

6.2.5. Site Specific Safety Plan (Due fourteen (14) calendar days after award)

6.2.6. Site quality control plan (Due fourteen (14) calendar days after award work)

6.2.7. Project Schedule (Due fourteen (14) calendar days after award)

6.3. All submittals shall be in Portable Document Format (PDF) format as well as hard copies as required.

7. Training:

7.1. All persons entering ANAD as part of this contract shall complete the following training prior to commencing any work on depot.

7.2. Resource Conservation and Recovery Act (RCRA) Training. ANAD Risk Management Office (DRK) will conduct RCRA training for each Contractor personnel working on site. This training is required annually. This training covers the latest rules for managing the site’s hazardous waste from cradle to grave. Proof of completion of training shall be kept on site and presented upon request to the DRK representative.

7.3. All Contractor personnel working on site must have attended DRK's environmental briefing (RCRA training) within the last year.
