About this file

The document is a Past Performance Questionnaire (PPQ) for the Veterans Health Administration (VHA) Network Contracting Office 20, used to evaluate a contractor's performance for a firm-fixed-price construction project to replace a chiller. The form is designed to be completed by a client evaluator and covers multiple performance dimensions including quality, schedule/timeliness, communication, management/personnel/labor, continuity of personnel, cost/financial management, and subcontract management.

The PPQ provides a structured method for rating contractor performance using adjectival ratings ranging from Outstanding to Unsatisfactory, with specific definitions for each rating level. The evaluator is asked to provide ratings and comments across seven key performance areas, with the ultimate goal of assessing whether the contractor would be recommended for future work. The completed questionnaire is intended to be incorporated into the contractor's SF330 submission and is not to be directly submitted to the contracting office.
