About this file
This is a Performance Assessment Questionnaire template used by the Department of the Army's 413th Contracting Support Brigade for evaluating offeror past performance in support of RFP W912CN-26-R-A006. The questionnaire is sent to past customers or points of contact identified by offerors to provide candid assessments of contractor performance on relevant federal, state, local government, or commercial contracts comparable to NAICS 561730.
Respondents must return completed questionnaires to the Contracting Officer no later than 15 days after receipt of the request or the Proposal Due Date, whichever occurs first, with submissions directed to nicholas.b.zehr.civ@army.mil, monica.l.napoleon.civ@army.mil, and martin.a.celino.civ@army.mil. The questionnaire contains three parts: Part I requires offerors to provide contract identification details including contractor name, address, program title, contract number, contract type, award date, completion date, nature of effort, and total contract value, along with offeror representative identification; Part II requires the point of contact respondent to rate performance on six evaluation criteria (compliance with specifications and workmanship standards, project management effectiveness, timeliness of performance, cost forecasting and control for cost reimbursement contracts, commitment to customer satisfaction, and general comments) using a binary Acceptable/Unacceptable rating system with required rationales; Part III requests return of the completed questionnaire. Rationale must be provided for each response, and respondents may identify other relevant information sources. Final ratings collapse into Acceptable or Unacceptable categories, with Acceptable defined as performance meeting contractual requirements with minor or no problems and effective corrective actions, and Unacceptable defined as performance not meeting requirements with serious problems and ineffective or unidentified corrective actions.
