About this file
This is a Past Performance Reference Form template for quoters responding to a Request for Quotation (RFQ) for an Axion BioSystems Maestro Pro Microelectrode Array (MEA) System issued by the FDA's Center for Biologics Evaluation and Research (Solicitation No. CBER-2026-135659). Quoters must submit up to three past performance references demonstrating recent and relevant experience with laboratory instrumentation, scientific equipment, microelectrode array systems, or electrophysiology equipment of similar scope, complexity, and magnitude.
The form requires quoters to document each reference's customer type (federal government, state/local government, university, hospital, commercial, or other), contract details including value and period of performance, and a comprehensive description of products and services provided. Quoters must address specific relevance factors including equipment similarity, technical complexity, onsite installation and configuration, user training, software and licensing, maintenance and technical support, and dollar magnitude comparability. The form includes sections for contract performance evaluation covering delivery schedules, technical conformance, installation completion, training satisfaction, and warranty performance. Quoters must also disclose any performance issues such as late deliveries, equipment rejection, installation problems, warranty failures, or corrective actions taken. The form includes optional CPARS performance rating information and requires authorization for government contact with references. Quoters without relevant past performance may complete Section 10 to indicate this circumstance and may receive a neutral past-performance rating under the Low Price Technically Acceptable (LPTA) evaluation methodology.
