About this file
This is a Past Performance Questionnaire (PPQ) template that must be completed by past performance references for the USPTO's Inventor's Assistance Center (IAC) Services solicitation #1333BJ25R00150007. The PPQ requires references to evaluate contractors across six key performance areas: Technical/Quality of Service, Schedule/Timeliness, Cost Control, Management/Business Relations, Small Business Subcontracting, and Regulatory Compliance.
The questionnaire uses a rating scale from Exceptional to Unsatisfactory (plus Not Applicable) and requires written narratives justifying any rating given. References must provide basic contract information including contract value, type, and period of performance. The completed PPQ must be signed by the reference and emailed directly to USPTO contract specialists Michael Spatuzzi and Kelly Gustafson. While this is a template document, it will be used to evaluate past performance for the IAC services procurement.

Rating Guidelines
Exceptional Past performance met and exceeded many of the contractual requirements to the customer’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few or no minor problems for which corrective actions taken by the contractor were highly effective
Very Good Past performance met and exceeded some of the contractual requirements to the customer’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few or no minor problems for which corrective actions taken by the contractor were effective.
Satisfactory Past performance met minimum contractual requirements. The contractual performance of the element or sub-element may have contained some minor problems for which corrective actions taken by the contractor were effective.
Marginal Past performance did not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflected a serious problem for which the contractor did not identify corrective actions. If the contractor proposed corrective actions, they were only marginally effective or were not fully implemented.
Unsatisfactory Past performance did not meet most contractual requirements and recovery did not occur in a timely manner. The contractual performance of the element or sub-element contained serious problem(s) for which the contractor's corrective actions were ineffective.
Not Applicable (Neutral) No information or did not apply to your contract.
