Addendum to FAR 52.212-1 – Instructions to Offerors – Commercial Items This acquisition will be solicited and awarded in accordance with Federal Acquisition Regulation (FAR) Part 12 -- Acquisition of Commercial Items and FAR Part 13 – Simplified Acquisition Procedures.

A Firm Fixed Price Contract will be awarded to the Offeror whose quote conforms to the solicitation requirements and represents the lowest evaluated price of proposals meeting or exceeding the acceptability standards for the non-cost factor of Technical Approach. Additionally, the Offeror must be deemed a responsible prospective contractor IAW FAR 9.1.

This acquisition is being solicited as a total small business set-aside.

INSTRUCTIONS TO OFFERORS: All quotes must include all data and information requested and must be submitted in accordance with these instructions. Any offeror who submits an incomplete package may be considered nonresponsive. All quotes are expected to conform to the solicitation provisions and be prepared in accordance with this section. To aid in the evaluation, the proposal shall be clearly and concisely written as well as being neat, indexed (crossed-indexed as appropriate) and logically assembled. All pages of each part shall be appropriately numbered and identified with the name of the offeror, the date, and the solicitation number.

Each Offeror shall submit:

a. Technical Approach Statement – Offerors are required to provide a detailed statement regarding their technical approach and capability to complete the requirements outlined in the scope of work. The Technical Statement shall explain how the work will be carried out in terms of the techniques, procedures, and schedule. The Technical Statement shall also include a detailed statement on experience, capacity, and expertise demonstrating the ability to perform the required tasks within the period of performance and to meet all goals and objectives on schedule.

b. Price Summary – Offeror’s pricing quote shall be submitted utilizing the Value Bid Schedule (Attachment in solicitation).

Addendum to FAR 52.212-2 – Evaluation – Commercial Products and Commercial Services BASIS FOR CONTRACT AWARD: The basis for the award will be Lowest Price Technically Acceptable (LPTA), and will consider the following evaluation factors:

a. Factor 1 – Technical Approach The Technical Approach Factor will either receive an acceptable or unacceptable rating based whether or not the offeror’s proposal meets the following requirements: a) testing plan outlines the correct number of sites, number of samples, schedule for sampling, and types of analysis specified in the SOW, b) in the case of a positive sample, outlines a plan for rush analysis, which includes the proper amount of testing in the response time as specified in the SOW, c) outlines a laboratory testing plan and other technical services such as generating and filing all applicable system reports as specified in the SOW, d) outlines a maintenance/treatment plan that meets the specifications in the SOW and ensures quality control of the potable system as specified in the SOW.

The offeror’s proposal addresses all requirements outlined above, and meets the specifications for all testing, maintenance, and treatment plans described in the SOW.
Unacceptable
The offeror’s proposal either does not address all the requirements outlined above, or the proposed plan for testing, maintenance, or treatment does not meet the specifications described in the SOW.

c. Factor 2 – Total Price: Price will not be scored or rated, but will be evaluated for reasonableness in accordance with (IAW) FAR 15.404-1(b).
