About this file

This is a Quality Assurance Surveillance Plan (QASP) for elevator maintenance and emergency response services at a healthcare facility. The document outlines the Government's approach to monitoring contractor performance and verifying compliance with the underlying Performance Work Statement (PWS).

The QASP establishes six key performance objectives with specific standards and acceptance quality levels (AQL). Preventive Maintenance requires 100% completion of monthly services for each elevator annually, monitored through monthly Contracting Officer's Representative (COR) review of maintenance logs and random inspections, with reperformance at no cost as the remedy for non-compliance. Emergency Response to entrapment must occur within 1 hour with a 95% AQL, verified through incident logs and COR verification, requiring a corrective action plan if deficient. Routine Repairs must be completed within agreed timeframes (typically 24–72 hours unless parts are required) at 95% AQL, monitored through work order tracking with reperformance or schedule adjustment as remedy. Healthcare Environment Compliance maintains a zero-tolerance standard for disruptions to patient care and violations of infection control requirements, with ongoing COR observation and immediate corrective action or possible site removal as remedy. Safety Compliance requires adherence to OSHA, ASME, NFPA, and facility safety requirements with zero-tolerance for violations, enforced through inspection reports and incident logs with possible work stoppage. Documentation requires 100% on-time submission of complete and accurate monthly maintenance and service reports, with invoice withholding as the remedy for non-compliance. All deficiencies must be corrected at no additional cost prior to Government acceptance.
