Attachment 3: Tailored FAR 52.212-1 INSTRUCTIONS TO OFFERORS—COMMERCIAL ITEMS
ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS—COMMERCIAL ITEMS Provisions that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available.
The following provisions are incorporated into 52.212-1 as an addendum to this solicitation:
ADDITIONAL INSTRUCTIONS TO QUOTERS:
All quoters shall submit the following proposal volumes in response to this solicitation. One complete set of volumes shall be clearly marked and emailed to the Contracting Officer (CO) at the following address no later than 22 March 2021, 4:00 PM Eastern Time:

QUESTIONS: Any questions regarding this solicitation must be submitted via email to the CO no later than 16 March 2021.
1. VOLUME I - TECHNICAL CAPABILITY
The quoter shall demonstrate understanding of the requirements of the Performance Work Statement and produce a quote that will meet those objectives. The quote will be evaluated based on the qualifications, experience and education of the proposed providers in addition to the contractor’s approach to meeting the needs of the performance work statement.
The quoter shall provide the following information in this volume:
· Identify physical address where services will be performed;
· Provide the following for all key personnel identified in the Key Personnel table in PWS Section 4: copies of applicable licenses/certifications and copies of CV’s/Resumes detailing work history and education;
· Quoter shall provide their process for clinical competency review of counselors, including documentation of reviews.
· Describe the quoter’s method of scheduling and managing qualified personnel;
· Clearly but briefly describe quoter’s contingency plan relating to qualified candidate availability.
2. VOLUME II – PAST PERFORMANCE INFORMATION
The quoter shall provide past performance information for evaluation. The quoter shall provide a list of up to three (3) recent (performed during the past (3) years from the date of issuance of this solicitation) contracts (Government and Commercial), performed by the quoter. This list shall include the complete name, address, and point of contact with telephone number for each contract. The list shall also include the relevant (similar or identical in complexity and performance) types of services provided, the dates of the contracts, and the dollar value of each. Information on problems encountered on any identified contract and quoter’s corrective action(s) may be included.
Note: A quoter with no past performance experience will be considered neutral and not a leading candidate.
3. Volume III – Contract Documentation
a) Completion of key personnel section of PWS (attachment 1), and completed and signed copy of QASP (Attachment 2).
b) Quoter’s representations and certifications in clause 52-212-3 or notice that these are available electronically via the Online, Representations and Certifications Application (ORCA) website.
c) COVER LETTER: The quoter’s proposal shall include a cover letter signed by an individual authorized to commit the company to the proposal. The cover letter shall identify all enclosures being transmitted as part of the proposal. The letter shall reference the solicitation number and acknowledge that it transmits an quote in response to the solicitation. The cover letter shall state:
1. Commercial and Government Entity (CAGE) number,
2. DUNS Number,
3. Taxpayer Identification Number (TIN),
4. Names and telephone numbers of persons authorized to conduct negotiations, as well as the names of the official authorized to bind the quoter’s organization shall be clearly identified, and
5. Indicate if firm qualifies as a small, small-disadvantaged, woman owned, HUBZone, veteran owned small business, or service-disabled/veteran-owned small business. * *In order to receive credit for being a VOSB or SDVOSB, the quoter shall provide a statement of compliance that it qualifies as a SDVOSB or VOSB in accordance with VAAR 852.215-70, “Service-Disabled Veteran Owned and Veteran Owned Small Business Evaluation Factors.” To receive some consideration under this evaluation factor, the quoter must state in its proposal the names of SDVOSB(s) and/or VOSB(s) with whom it intends to subcontract and provide a brief description and the approximate dollar values of the proposed subcontracts.
4. Proposed Price The quoter shall provide unit and total pricing for the base period and all option periods beginning on page 4 of the Combined-Synopsis Solicitation document. No separate price volume is required.
5. Exceptions to Terms and Conditions
To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows:
"The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."
OR
"The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"
Quoters shall list exception(s) and rationale for the exception(s), if any.
