STATEMENT OF NEED
Provision of Data and Analysis for the HEROES Program Outcome Toolkit
1.0 SCOPE
The Advanced Research Projects Agency for Health (ARPA-H) Resilient Systems Office seeks to support the procurement of data for the HEealthcare Rewards to Achieve Improved OutcomES for all (HEROES) program Outcome Toolkit. The HEROES Program aims to provide tools and technical support to organizations to improve health care in large, geographically defined populations through implementation of novel technologies and strategies, track changes in quantifiable outcomes metrics in near real-time, and develop economic incentives that reward improvements via a sustainable, scalable economic model. The incentives, or outcomes payments, will depend on HEROES Program participants achieving transparent and measurable pre-determined outcomes known to have major impacts on health.
The HEROES Program focuses on four outcomes that were selected for their continued health burden due to significant morbidity and mortality despite the availability of effective evidence-based interventions to prevent them or reduce their severity. The outcomes include:
1. Maternal Health
2. Heart Attack and Stroke Risk
Data for these outcomes will feed into a dynamic, near-real time Outcome Toolkit. This technology will allow Health Accelerators (HEROES program participants) to calculate the baseline rate of selected health outcomes for a targeted geography. This tool will enable the targeted focus of interventions as well as the ability to evaluate improvements in outcomes in near-real-time. In addition to tracking improvements, the Outcome Toolkit will be used to identify a matched control-group and evaluate the Program’s effectiveness using a rigorous study design.
The success of the HEROES program and the specific reward payments made to performers will be dependent on high-quality and timely data for the relevant health outcomes that the program is addressing (e.g., data on geographically specific rates of atherosclerotic cardiovascular disease and associated 10-year risk scores will be used to track a performers progress at reducing heart attack and stroke risk in targeted geographies and will govern the success-based payments that the performer may receive).
ARPA-H wishes to procure datasets relevant to the following health outcomes in support of the program’s objectives.
Maternal Health: Severe Obstetric Complications spanning birth and a 60-day post-partum period.
Heart Attack and Stroke Risk: Atherosclerotic cardiovascular disease and associated 10-year risk scores calculated from raw data.
2.0 LOCATION
Not applicable
3.0 PERFORMANCE PERIOD
Data and associated calculations performed on the data are requested to span 30 August 2024 – 29 August 2026 with a 12-month base plus 12-month option and a five-month option exercise under FAR 52.217-8.
4.0 REQUIREMENTS
The following are the minimal requirements from ARPA-H to effectively support the HEROES program’s data toolkit.
Data variables for Severe Obstetric Complications (SOC)
Data variables for SOC as defined by CMS (Severe Obstetric Complications | eCQI Resource Center (healthit.gov)) o Acute myocardial infarction o Aortic aneurysm o Cardiac arrest/ventricular fibrillation o Heart failure/arrest during procedure or surgery o Disseminated intravascular coagulation o Shock o Acute renal failure o Adult respiratory distress syndrome o Pulmonary edema/Acute heart failure o Sepsis o Air and thrombotic embolism o Amniotic fluid embolism o Eclampsia o Severe anesthesia complications o Puerperal cerebrovascular disease o Sickle cell disease with crisis o Blood transfusion o Conversion of cardiac rhythm o Hysterectomy o Temporary tracheostomy o Ventilation
Above data should be associated with a de-identified unique patient ID and collated with other claims or health system encounters for that patient during the delivery hospitalization and extending 60 days postpartum. Note: This is a modification to the measure as currently specified.
Data variables for Atherosclerotic Cardiovascular Disease and Associated 10-year Risk Scores
Specific data variables for all individuals aged 40-70 years of age.
o Blood pressure (systolic and diastolic values) o Total cholesterol o HDL cholesterol o LDL cholesterol o Smoking history (Yes/No) o Diabetes history (Yes/No) o Hypertension treatment (Yes/No) o Statin medication (Yes/No) o Aspirin therapy (Yes/No)
Utilizing the variables above, the vendor will calculate a 10-year ASCVD risk score using the American College of Cardiology/American Heart Association Risk Estimator Plus. This data element will be included as part of the data submission. The variable will be reported as a percent.
o The risk score calculations will incorporate the dynamic nature of individual risk scores by assessing the initial (or baseline) risk score as well as secondary (longitudinal) calculation of risk scores after a patient has applied various interventions.
o Baseline and longitudinal ASCVD risk scores should be associated with a de-identified unique patient ID to enable accurate assessment of changes in risk over time.
Additional data variables specific to the HEROES program Admissions to inpatient hospitals, psychiatric centers, and substance use treatment centers for mental health conditions (e.g., suicide, self-harm, substance use, interpersonal violence, severe persistent mental illness).
Other data variables may be added throughout the term of this agreement.
Data granularity, coverage, and metadata
Where available, the following demographic information / metadata should be provided in addition to the specific outcome metrics and should be associated with unique de-identified patient IDs. The Vendor should follow the OMB approved race and ethnicity categories.
o Age o Sex o Gender o Race/ethnicity o Clinical profile (e.g., immunocompromised, presence of chronic conditions) o Insurance status and type (Commercial, Medicare, Medicaid, Uninsured) o Median Income
Spatial granularity: Minimum Zip3 (first three digits of zip code) o Vendors will modify the geographic level of data reporting based on appropriate privacy standards.
Coverage: All US
Data history and prospective timeline
Maintenance and refreshes of the historical data already provided in the previous contract.
Ongoing monthly datasets (including metadata) for each health outcome over a 4-year period.
Data Format & Transmission
All data should be in a consistent, mutually agreed upon machine-readable data format (either long or wide format).

Vendor will provide all deliverables in a timely manner to recipients designated by
ARPA-H, potentially including third-party service providers contracted by ARPA-H (e.g., for the purposes of embedding data into the public-dashboard).
Vendor will ensure deliverables are transmitted or otherwise made available in a manner that is compliant with all appropriate security and privacy standards (e.g., via Secure File Transfer Protocol or download from secure platform).
5.0 OTHER
For shared understanding, vendor will not be providing any Personal Identifiable Information (PII) or Protected Health Information (PHI).
