About this file

This is a Work Order Contract (WOC) template executed by the State of Oregon, acting through its Oregon Health Authority Behavioral Health Division (BHD), with selected providers to deliver forensic evaluation services under a master Price Agreement. The contract authorizes providers to conduct forensic competency evaluations pursuant to Oregon Revised Statute 161.370. The WOC specifies that providers shall perform no more than a designated number of forensic evaluations during the contract term, with each evaluation initiated by individual request from BHD identifying the specific individual and location. The contract becomes effective upon execution by both parties and receipt of all applicable state approvals, and expires on a date to be specified in the final WOC. Providers will not receive payment for services rendered prior to necessary state approvals.

Compensation is structured on an hourly rate basis as established in the underlying Price Agreement, with a maximum not-to-exceed amount specified for each WOC. Providers must maintain insurance coverage as required in Exhibit B of the Price Agreement and submit certificates of insurance within 30 days of contract execution. The certification section requires providers to confirm compliance with the Oregon False Claims Act, state and local tax laws, non-discrimination requirements, and various federal exclusion lists. Providers must also verify they are not subject to backup withholding and provide accurate Federal Identification Numbers or Social Security Numbers, with notification required within 10 days of any changes to this information.
