About this file

This file is a Past Performance Questionnaire template designed to evaluate contractor performance on federal contracts. The questionnaire captures evaluation data through a standardized Performance Rating Form that collects basic contract information (company name, address, contract number, period of performance, contract value, and contract type) along with the evaluator's details and contact information.

The evaluation framework assesses eight performance elements across four categories using a three-tier confidence rating system (High, Medium, or Low Confidence). Technical Performance is measured through quality of technical approach and understanding of requirements. Timeliness of Performance evaluates effective resource utilization and schedule adherence. Quality/Customer Satisfaction assesses commitment to customer service, business behavior, and communication effectiveness with government officials, technical representatives, end users, and other stakeholders. Cost Control measures planning, monitoring, budget adherence, and cost-effective implementation of changes. Each element allows evaluators to rate performance as Outstanding, Good, or Acceptable, with space for detailed comments explaining ratings. The form concludes with a yes/no question on contractor rehire likelihood and an additional comments section. This template serves as an appendix to the Regional Innovation Clusters solicitation (47QACA26R0295) issued by the Small Business Administration's Office of Investment and Innovation to solicit contractor services for creating regional public-private partnerships focused on technology innovation and economic growth.
