Required Service Performance Standard
Staffing Management Maintain 100% of required assigned staffing levels. Contractor's staff will contact Supervisor if unable to report or will be late, and contractor will replace staff and notify COR of overall staffing level.
Professionalism Conducts business in a professional manner and in compliance with policy, including attire guidelines. Contractor will greet all staff, patients, and visitors who enter the facility grounds. All interactions with personnel are done with dignity and respect.
Documentation Compliance Timecards will be submitted weekly to ensure prompt payment. All documentation requirements need to be submitted weekly to the COR.
Safety and Security Safety and Security incidents must be reported using Patient Safety Reporting tool, ISSO and COR must also be notified immediately.
Privacy, Confidentiality, and HIPAA Contractor is aware of all laws, regulations, policies, procedures relating to Privacy, Confidentiality, and HIPAA and complies with all standards.
Timely and Accurate Invoicing Contractor shall provide itemized invoicing with the correct dates, contractor name and address, contract number, PO number, itemized CLINs, and total for each billing period.
Mandatory Training Contractor must complete all mandatory training before contract award date.
Training must remain current and cannot lapse.
Acceptable Performance Levels Method of Surveillance Corrective Action
No more than 2 deviations per Month.
Invoice, Notification from contractor, observation.
Contractor will discount monthly invoice by 5% for any month that logged more than 2 deviations.
No more than 1 deviation per month. No significant/aggravated deviations as defined by the Contracting Officer in concert with the
POC/COR.
Visual observation, reports from facility staff.
VA will request personnel be replaced at Contractor's expense, and without a lapse in services provided
No more than 1 deviation per 90 days.
Weekly reports, visual observation, compliance checks.
Contractor will discount monthly invoice by 3% for any 90 day period that logged 2 or more deviations.
100% of all incidents reported immediately (within 4 hours).

CO notification.
Zero breaches of privacy or confidentiality.
Visual observation, compliance checks.
CO notification.
All invoices uploaded into Tungsten monthly in arrears, no more than 2 deviations a quarter.
Certifying official.
Invoices will be refused and contractor will need to resubmit with corrected invoice.
100% compliance in training requirements - any lapse must be corrected within 5 business days of notification.
COR, TMS reports VA will request contractor to replace employee at contractor expense, and without a lapse in services provided
