About this file

This document is a Past Performance Questionnaire (PPQ) issued by the 628th Contracting Squadron at Joint Base Charleston for a solicitation to procure customized Mobility War Trailers for the 437th Maintenance Group/AMXS. The PPQ is designed to validate a potential contractor's past performance by requesting detailed feedback from previous clients on their performance across multiple evaluation criteria, including ability to meet quality standards, on-time delivery, communication responsiveness, engineering capabilities, and safety performance.

The questionnaire includes a rating scale from Exceptional (E) to Unsatisfactory (U) for various performance dimensions, with options to provide detailed explanations for ratings. Key sections require the reference to assess the contractor's overall performance, confirm whether they would award another contract, and disclose any contract terminations. The document specifies a response deadline of 12 pm EST on 25 April 2025, with completed questionnaires to be emailed to Morgan Hart and Nicole Alvarez at their Air Force email addresses. This PPQ will be used as part of the proposal evaluation process to determine the contractor's capability to perform on the proposed Mobility War Trailer contract.
