About this file
This is a Past Performance Assessment Questionnaire issued by the U.S. Army Mission and Installation Contracting Command (MICC), Theater Support Center (TSC), in support of Request for Proposal W51EW7-26-R-A004 for Base Test Support Services (BTSS) Follow-on. The document requests evaluation of offeror past performance on recent and relevant government contracts. Respondents must complete the questionnaire and return it to Contracting Officer Ingrid V. Smith (ingrid.v.smith.civ@army.mil) and Contract Specialist Avangela Y. Ligons-Sutton (avangela.y.ligons-sutton.civ@army.mil) no later than five business days after receipt or by 4:30 PM EDT on April 8, 2026, whichever occurs first.
The questionnaire requires evaluators to rate offeror performance across six evaluation criteria using a four-level confidence scale: Substantial Confidence, Satisfactory Confidence, Limited Confidence, or No Confidence. The evaluation categories assess compliance with specification requirements and workmanship standards, project management and subcontractor control effectiveness, timeliness of service and product deliverables, cost forecasting and control, commitment to customer satisfaction, and comparison to BTSS scope. Part F specifically requests identification of relevant contract experience including Top Secret security clearance requirements, contractor-provided personnel for management and logistics functions, phase-in and transition activities within 30 days or less, field test services, government-furnished property and facilities management, diverse operational environments from CONUS to OCONUS locations, and flexible rapid personnel expansion and contraction capabilities. Respondents must also provide rationale for each rating and identify other relevant past performance points of contact.
