About this file
This is an Indian Affairs Indian Economic Enterprise Representation Form used for the Buy Indian Act solicitation 140A2326Q0104 for the UTVs, Haskell project. The form serves as a self-certification document that offerors must complete to demonstrate eligibility as an Indian Economic Enterprise (IEE) as defined in the Department of the Interior Acquisition Regulation (DIAR) Part 1480.
Offerors must meet IEE requirements at three critical junctures: when submitting an offer in response to the written solicitation, at the time of contract award, and throughout the full contract term. The form requires offerors to provide the name of the Federally Recognized Tribal Entity, the Legal Business Name, the Unique Entity ID (UEI), and the name of the 51% (or greater) owner of the IEE firm. A certifying signature and printed name from an authorized representative must accompany the submission. Contracting Officers retain the authority to request additional eligibility documentation at any point during the acquisition lifecycle. False or misleading information submitted in connection with a Buy Indian Act set-aside award constitutes a violation of 18 U.S.C. 1001, while false claims during contract performance are subject to penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287.
