About this file

This document is an Attachment 3 form for Terms and Conditions Acceptance related to a state or local court contract opportunity. The attachment provides bidders with three options for responding to the contract's standard terms and conditions: (1) agreeing to current contract terms with a potential notice section update, (2) accepting the Court's standard terms without exception, or (3) proposing specific exceptions or modifications to the standard terms. Bidders selecting option 3 must submit a red-lined Word document tracking proposed changes and provide a written explanation for each modification, with the understanding that the Court retains sole discretion to determine if proposed changes constitute a material deviation that could render the bid non-responsive.

The document also includes a mandatory list of 16 certification clauses that could cause bid rejection if waived, covering areas such as Child Support Compliance, Darfur Contracting Act, Domestic Partners and Gender Discrimination, Antitrust Claims, Audit Requirements, and DVBE (Disabled Veteran Business Enterprise) commitments. The form requires an authorized company representative to sign, certifying their legal ability to bind the bidding organization, and provides space for company name, signature, execution date, and the printed name and title of the signing individual.
