About this file

This document is a Work Request Unscheduled Repair Authorization Request form designed to detail and gain approval for unplanned repair work. The form requires contractors to provide comprehensive information about proposed repairs, including a scope of work description, parts/materials needed (both OEM and non-OEM), associated costs, labor requirements, and markup rates. The document mandates detailed documentation for parts pricing, specifications, and cost justification, with explicit requirements that contractors provide proof of fair and reasonable pricing for materials.

The form includes four key sections: Scope of Work & Repair Description, Contractor Approved Markup on Materials, Labor Details, and Approval and Authorization. Before any work can commence, the request must receive approvals from the Project Manager (PM), Contracting Officer's Representative (COR), and Contracting Officer (KO), who will verify sufficient funds, validate labor hours, confirm material specifications, and ensure all costs align with pre-approved contract rates. The form emphasizes strict documentation and cost transparency, requiring separate invoicing for parts, labor, and markup, with all rates matching the originally agreed contract terms.
