Quality Assurance Surveillance Plan (QASP)
Elevator Annual and Inspection Requirement Service Contract
1. Purpose
The purpose of this QASP is to ensure that the contractor performs the required elevator maintenance and inspection services in accordance with the terms and conditions of the contract, including all applicable regulations and standards.
2. Scope
This QASP applies to all activities, documentation, and deliverables associated with the Elevator Annual and Inspection services provided by the contractor.
3. Roles and Responsibilities
• Contracting Officer (CO): Responsible for overall contract administration.
• Contracting Officer's Representative (COR): Monitors contractor performance, conducts inspections, and evaluates contractor deliverables.
• Quality Assurance Evaluator (QAE): Assists the COR in performance assessments, documenting contractor performance, and other tasks as assigned.
4. Performance Standards
The contractor must meet the following performance standards:
• Timeliness: Services are performed within the scheduled and required time frames.
• Quality: Services meet industry standards and comply with all safety and regulatory requirements.
• Documentation: Complete and accurate records of maintenance and inspections are maintained and submitted to the COR.
5. Performance Metrics
The following metrics will be used to evaluate contractor performance:
• Service Completion Rate: Percentage of inspections and maintenance tasks completed on schedule.
• Deficiency Rate: Number of deficiencies identified during inspections relative to the total number of inspections performed.
• Response Time: Time taken to respond to and correct any identified deficiencies.
• Documentation Accuracy: Completeness and correctness of maintenance and inspection records.
6. Methods of Surveillance
• Direct Observation: COR or QAE conducts on-site inspections of the contractor’s work.
• Periodic Inspections: Scheduled inspections are performed to verify compliance.
• Review of Documentation: Regular review of the contractor’s maintenance and inspection reports.
• Customer Feedback: Collection of feedback from facility users.
7. Performance Measurement
• Performance will be measured and documented using the following tools:
• Checklists and Inspection Forms: For direct observation and periodic inspections.
• Performance Evaluation Reports: Summarizing performance metrics and overall contractor performance.
• Customer Feedback Forms: For documenting user feedback.
8. Performance Reporting
The COR will compile performance data and provide reports to the CO on a monthly/quarterly basis. These reports will include:
• Summary of performance metrics.
• Detailed findings of inspections and reviews.
• Recommendations for any corrective actions.
9. Contractor Notification and Corrective Actions
If performance standards are not met, the contractor will be notified in writing. The notification will include:
• Specific deficiencies.
• Required corrective actions.
• Time frame for correction. Contractor performance will be re-evaluated after the correction period.
10. Documentation and Record-Keeping
All records related to QASP activities, including inspection reports, performance evaluations, and communication with the contractor, will be maintained for the duration of the contract.
