PERFORMANCE WORK STATEMENT (PWS) for PREVENTATIVE and CORRECTIVE

MAINTENANCE: HOIST INSPECTIONS, ANNUAL LOAD TEST FOR RIGGING GEAR FOR

DLA DISTRIBUTION OKLAHOMA CITY-TINKER AIR FORCE BASE

1.0. Scope

This Performance Work Statement (PWS) outlines requirements for preventative and corrective maintenance, repair, periodic inspections, load testing and certification services for Hoist, Rigging Gear to include Rigging Hardware, ID Tags, Wire Rope, Chain, Synthetic Slings, Spreader Bars, and Caps, etc. Automated Material Handling and Material Handling Equipment operated by DLA Distribution Oklahoma City (DDOO) at Tinker Air Force Base, Oklahoma. The objective is to ensure safe and compliant operation of Hoist and Rigging Gear in accordance with (IAW) OSHA, ASME, ANSI, and DoD standards.

2.0. Scope of Work

The contractor shall provide all labor, tools, equipment, materials, transportation, and supervision necessary to perform: DLA DDOO will provide the MHE Support to assist with the contractor’s inspections.

Yearly Inspections of one (33) Hoist Certified Load Test for the following equipment:

Thirty-Three (33) Hoist Yearly Inspections for the following Rigging Gear equipment to include Rigging Hardware, ID Tags, Wire Rope Chain, Synthetic Slings, Spreader Bars, and Caps

2.1. All work should be performed in accordance with the latest applicable federal state and local regulations including but not limited to OSHA 29 CFR 1910.179, ASME B30 series, and relevant DoD and Air Force safety directives.

2.2 Labor/Scope Inspections: Yearly Inspection – 33 Hoist

Provide all management, qualified labor, tools, and supplies to inspect 33 Cranes/Hoist as per the list provided by Tinker.

Inspections included Safety items, Electrical components, Rail alignment and support, Wire rope/Chain condition, Hook, and Latch, Drives and gear boxes.

Furnish detailed report of inspection results and adhesive label to include equipment identifying information, test date, test type results, and certification of inspection, adhere label to equipment.

Provide detailed repair estimates for deficiencies identified during the inspection.

2.3 – 33 Hoist

Provide all management, qualified labor, tools, and supplies to load test 33 Hoist as per the list provided by Tinker.

Provide certified weights and rigging.

Perform pre-test inspection of all 33 Hoist listed.

Perform load tests at not less than 100% capacity and not more than 125% of rated capacity.

Furnish detailed report of inspection and load test results and adhesive label to include equipment identifying information, test dates, test type, load applied, results and certification of load test, adhere label to equipment.

Load tests may be performed in conjunction with yearly inspection.

2.4 Labor/Scope Inspections: Yearly Inspection –

Provide all management, qualified labor, tools, and supplies to inspect Rigging Gear as per the list provided by Tinker.

Inspections included Safety items, Rigging Hardware, ID Tags, Wire Rope, Chain, Synthetic Slings, Spreader Bars, Caps, etc.

Both Visual Inspection & Pull Test o We will make (3) Different Pick-Ups and Drop Offs to/from Tinker of all listed rigging gear to perform Pull Test.

o Spreader Bar: Pull Test required at 125% Capacity.

Furnish detailed report of inspection results and adhesive label to include equipment identifying information, test date, test type, test type, results, and certification of inspection, adhere label to equipment.

Defective rigging equipment shall be removed from service. Not to be used without affixed, legible identification markings, required by OHSA paragraph 1926.251(a)(2)(i).

2.5 All work should be performed in accordance with the latest applicable federal state and local regulations including but not limited to OSHA 29 CFR 1910.179, ASME B30 series, and relevant DoD and Air Force safety directives.

3.0 Task – Operations, Sustainment and Maintenance

The Government’s overall functional and technical requirements for operational sustainment and maintenance support for Bridge and Mobile Crane are listed in this section. The Government’s specific technical requirements include by providing the requirements IAW Federal, State, Local, DoD, and DLA policies, procedures, and statues.

The Contractor shall provide operations, sustainment, and maintenance support including mechanical Preventative Maintenance (PM), Corrective Maintenance (CM), and technical services and support. Specifically, the Contractor shall provide the personnel, equipment, replacement spares, mechanical hardware including for a fully operational AMHS and MHE equipment, but not limited to the inspection, maintenance, services.

o Preventative Maintenance (PM) includes a recurring set of scheduled services and actions which the Contractor shall provide to keep the equipment fully operational and compliant o Corrective Maintenance (CM) is the non-recurring unscheduled repairs or services required to make the equipment fully operational.

3.1 Sustainment

The Contractor shall develop and implement a Sustainment Plan for all equipment IAW with the original equipment manufacturer and/or Federal/DoD/DLA policies and procedures. The Sustainment Plan includes quarterly inspections, repair, and replacement. The Contractor shall provide the Sustainment Plan which includes the quarterly inspections, and annual load tests for the equipment, the PM schedule and CM approach to the COR for approval.

The CM approach should include:

o Certifications of testing equipment and other pertinent information are needed to quickly diagnose and make repairs.

o Plans and documentation for equipment testing and/or diagnoses o Identify critical equipment operations and implement strategies to prevent potential failures or equipment downtime from creating a critical work stoppage.

o Plan to respond to take corrective action to resolve the problem/mechanical emergency repairs by the Contractor or the Government during normal duty hours.

3.2 Preventative Maintenance/ Annual Load Testing, and Quarterly Inspections

The Contractor shall perform Preventative Maintenance (PM) services on all equipment which includes, but is not limited to inspection, positioning, testing, cleaning, lubrication, adjustment, and calibration to verify proper equipment operations. The Contractor shall minimize malfunctions, breakdowns, and deterioration of equipment following industry, Original Equipment Manufacturer (OEM) and Government best practices. The Contractor shall perform PM in accordance with agreed upon schedule. The Contractor shall submit and keep up to date a schedule for all PM tasks.

Any modifications, changes, add-ons, or deletions shall be submitted to the COR for approval and once approved and implemented and provided to the site.

o The Contractor will provide an Annual and Quarterly Preventative Maintenance (PM) Schedule and within five (5) days of exercised Option Year dates each calendar year and perform PM IAW to approved schedule.

o All replacement parts, materials, environmental compliance, and Contractor personnel transportation used for PM shall be included in the contract unit price for PM services.

3.3. Corrective Maintenance (CM) – Service Call(s) Work

Service Calls Requirements and Procedures – Service calls are defined as emergent maintenance and repair work requirements which are identified by building occupants. The Contractor shall perform service work as necessary to determine the cause of system and equipment malfunctions, eliminate the cause(s), and restore the equipment to working condition.

Service Calls Requests – The On-Site Representative will advise the Contractor by phone or email of all service requests received during and after regular working hours. A description of the problem or requested work, date and time received, location and other appropriate information will be placed on the ticket.

Service Calls Responses – The Contractor shall have adequate procedures for receiving and responding to service calls during the Duty Day. A telephone number shall be provided by the contractor for receipt of all service calls. All telephone calls shall be answered within 30 minutes by an individual familiar with the Contractor’s work, control procedures, and the terms and conditions of this contact. Service calls shall be considered received by the Contractor at the time and date the telephone call is placed, or email sent. The Contractor shall respond immediately and must be on the job site and working within 24 (24) hours after receipt of a service call. The Contractor shall work continuously without interruption and shall resolve the issue before departing the job site.

Completed Service Calls – Within one (1) working day after completion of each service call, the Contractor shall add the following information to the work authorization form and return to the Budget Resource Officer o Description of work completed.

o Brief description of material and parts used, including quantities.

o Date and Time work began o Dat and time work was completed o Hours of labor (by trade) expended.

o Signature or initials of the Contractor’s craftsman performing the work (or supervisor) indicating that the work has been completed.

3.4 Service Calls Materials and Equipment

The Contractor shall have access to the equipment necessary to support service call work requirements. Lack of availability of equipment shall not relieve the Contractor from the requirement to complete service call work within the time limits specified above. Replacement parts are required in the performance of CM shall be invoiced to the Government IAW the terms and conditions of the contract (FAR52.212-4 Alternate I).

3.5 General Acceptance Criteria

General quality measures, as set forth below, will be applied to each work product received from the Contractor under the resultant task order. These performances are defined as:

o Quality o Accuracy – Work products shall be accurate in presentation, technical content, and adherence to accepted elements of style o Clarity – Work products shall be clear and concise. Any/all information shall be easy to understand and be relevant to the supporting narrative.

o Consistency to Requirements – All work products must satisfy the requirements of this Performance Work Statement (PWS).

o Format – Work products shall be submitted in hard copy (where applicable) mutually agreed upon prior to submission. Hard copy formats shall follow any specified Directives or Manuals.

o Timeliness o Work products shall be submitted on or before the due dates determined/specified between the Contractor or submitted IAW a later scheduled date determined by the Government.

4.0 Quality Control Plan (QCP)?

The Contractor shall submit a QCP within ten (10) business days of contract start date to the COR. This plan shall describe the Contractor’s methodology of compliance with the Deliverables and Performance Requirements Summary outlined above. The COR will notify the Contractor of concurrence or required modifications to the QCP within ten (10) business days of receipt. The Contractor shall make appropriate modifications within five (5) business days of the COR’s notification and provide a revised QCP within five (5) business days.

5.0 Safety and Standards

The Contractor shall be responsible for compliance with all local, state, and federal laws and codes

The Contractor shall follow all safety regulations, industry standards and practices The Contractor shall always be responsible for and maintain a safe and clean work area. The

Contractor shall remove all refuse and trash daily and ensure not to block and fire egress walkways.

6.0 Location of Services

All services shall be performed at:

o DLA Distribution Oklahoma City, Tinker Air Force Base, Oklahoma 73145

Specific equipment locations within the base will be coordinated upon contract award.
