About this file

This document is a past performance questionnaire for a solicitation issued by the Department of the Treasury's Office of the Comptroller of the Currency seeking home price indices data. The solicitation number is 2031JW23Q00095, with responses due no later than July 26, 2023. References are instructed to use a rating scale of excellent, good, satisfactory, poor, and unsatisfactory when evaluating the contractor's past performance. The questionnaire also asks references if the contractor has received any cure notices or letters of direction, if the reference would exercise options or award to the contractor today, and provides space for additional comments.

To Quoter’s Reference:

The Office of the Comptroller of the Currency (OCC), Office of Management – Acquisition Management requests your assistance in providing past performance information on the Contractor (a prospective “Quoter” under our Solicitation) and corresponding contract listed herein. Please complete this Questionnaire in accordance with the instructions set forth immediately below.

Acquisition Management agrees to protect your information as For Official Use Only/Procurement Sensitive Information consistent with FAR 3.104, as noted in the header on each page of the Questionnaire package.

Instructions for Quoter’s Reference:

1. If you are providing a reference based upon a Federal Government contract, then please complete the attached Past Performance Questionnaire regarding the Quoter based upon your most recent Contractor Performance Report (CPAR) prepared in accordance with FAR 42.15; provided, you do not actually enter your CPARs into the Contractor Performance Assessment Report System (CPARS) or the Past Performance Information Retrieval System (PPIRS). Alternatively, if you are providing a reference for a non-federal Government contract, then please proceed with completing the past performance questionnaire.
