About this file

This is a Past Performance Questionnaire template designed for NOAA to evaluate contractor performance on the West Coast Region Marine Mammal Hotline Service contract (Solicitation Number 1305M326Q0082). The questionnaire is completed in two stages: offerors fill out Section II with contract details and submit it to government assessors, while assessors verify the information and complete Sections III and IV based on their direct experience administering the contract. Completed questionnaires must be submitted to madison.vogeler@noaa.gov and jenna.taulman@noaa.gov by the solicitation closing date.

The questionnaire collects historical contract information including contractor name and address, contract number, contract type, contract value, period of performance, and project description. Assessors identify their role in contract administration (such as Procuring Contracting Officer, Administrative Contracting Officer, Contracting Officer's Representative, or Technical Project Lead) and provide an overall performance rating using a five-tier scale: Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable. The ratings are based on the contractor's performance against contractual requirements, quality and timeliness of delivery, cost management, inter-party communications, proactive management, and customer satisfaction. The template is a standard government instrument for source selection decisions and is marked "For Official Use Only."
