About this file

This is an Organizational Conflict of Interest (OCI) questionnaire form that must be completed by potential contractors to disclose any actual or potential conflicts of interest. The questionnaire contains 16 questions requiring respondents to disclose: prior involvement in the project; major business types; affiliates; involvement of executives and affiliated entities; energy industry connections; financial relationships with affected entities; income percentages from related arrangements; federal contract history; technology interests; access to sensitive information; and work on similar projects in the same geographic area.

The form requires detailed disclosures and explanations on attached pages for any "Yes" responses. It includes an annual updating requirement where contractors must file amended OCI statements within 30 days of becoming aware of any actual or potential conflicts during contract performance. The questionnaire must be signed by an authorized representative certifying the accuracy and completeness of all disclosures. This appears to be Attachment 2 associated with a Federal Energy Regulatory Commission (FERC) solicitation for Risk-Informed Decision Making Technical Support Services.
