ATTACHMENT 2 - STATEMENT OF WORK (SOW)

Bulk Lime and Fertilizer Application

1.0 INTRODUCTION:

Application of fertilizer and lime applied in bulk form to the ACP Post Closure Units (Burial Caps)

2.0 SCOPE:

The Application of Lime is to begin on/or after August 9, 2021 and to be completed by September 30th, 2021

Optional Item: The Application of Fertilizer is to begin on/or after March 29, 2022 and to be completed by April 30th, 2022

Optional Item: The Application of Lime is to begin on/or after August 9, 2022 and to be completed by September 30th, 2022

The Applicator will need to come by COR’s office for Remote Worker Map, Pager, List of Caps for Fertilizer Application and List of Caps with Fertilizer and Herbicide. The Application Truck will need to be inspected before use on the site.

3.0 PERSONNEL QUALIFICATIONS:

None, except as required by employer

4.0 GOVERNMENT-FURNISHED EQUIPMENT (GFE) AND MATERIALS:

1. Government Furnished Equipment:

a. Pager

b. Remote Worker Map

c. Post Closure Lock Key for Burial Cap

5.0 REGULATIONS/POLICIES:

Contractor will use specifications outlined in ATTACHMENT 3 - Specifications

6.0 Burial Cap Access:

The Contracting Officer’s Representative (COR) listed in paragraph 8.0 will assist with the Contractor’s entry into secure areas and inform the Contractor of security procedures. The Contractor shall comply with all security procedures.

7.0 TRAINING:

The Contractor is responsible for all training for its employees.
