About this file
This document is a Baseline Asbestos Assessment Survey for Building 500 at the Lake Baldwin Orlando VA Medical Center, conducted by Thunderbird-HRP JV, LLC in October 2024. The comprehensive survey identified multiple areas containing asbestos-containing materials (ACM), primarily on the first floor, including flooring materials, mastics, and duct insulation. Specifically, the assessment found approximately 5,028 square feet of mastic containing asbestos (5% Chrysotile) associated with vinyl floor tiles, and an additional 600 square feet of mosaic vinyl floor sheeting with asbestos in its backing.
The survey recommends that prior to any disturbance activities, confirmed and assumed ACM should be abated or provided with appropriate response actions. The report suggests treating any unsampled suspect materials as potentially containing asbestos until proven otherwise. Additionally, the survey recommends submitting a notification to the Florida Department of Environmental Protection at least 10 days before renovation/demolition, in accordance with state and federal regulations. The assessment was conducted following Veterans Health Administration (VHA) Directive 7714 and complies with OSHA and EPA asbestos management regulations.

EXECUTIVE SUMMARY
Baseline Asbestos Assessment Survey
Assessment Location: Lake Baldwin Orlando VA Medical Center: Building 500 – Main Clinic, located at
13800 Veterans Way, Orlando, Florida, 32083 (referred to herein as “the Site”)
Purpose of Assessment: The Veterans Health Administration (VHA) Directive 7714 (Asbestos
Management Program), dated April 6, 2017, calls for a survey following requirements of the Asbestos
Hazard Emergency Response Act (AHERA), known as a baseline survey. The purpose of the baseline survey is to identify, quantify, and sample suspect asbestos containing material (ACM) throughout the building. The survey was conducted in accordance with the Occupational Safety and Health
Administration (OSHA) and the Environmental Protection Agency (EPA) regulations. Non-suspect materials, which are defined as materials that are not expected to contain asbestos, were not sampled or analyzed as part of this survey.
Assessment Activities: The baseline survey for Building 500 was conducted on October 24th through
October 28th, 2024, by Thunderbird-HRP JV, LLC (Thunderbird-HRP) accredited asbestos inspectors. The survey included collecting representative samples of suspect asbestos-containing materials (ACMs), assessing their condition, and quantifying the materials and hazards present throughout the building.
Samples were submitted to Arrowhead Technologies, LLC, in Clearwater, Florida, a National Voluntary
Laboratory Accreditation Program accredited laboratory (NVLAP code 200703-0), for analysis of asbestos content by the Polarized Light Microscopy (PLM) method. A summary of suspect materials included in the asbestos survey, with those reported to be ACM (if applicable) indicated in bold, is included in Table 1.
Key Findings
As detailed in Table 1, the following is provided as a summary of materials containing asbestos:
The 1st floor of Building 500 contains multiple flooring materials and mastics reported to contain asbestos and are identified as ACM. Specifically, HA-34, HA-38, HA-54, HA-56, HA-106 and HA-144 have mastic containing asbestos (approximately 5,028 square feet) with the associated vinyl floor tile containing no asbestos. HA-55 is a mosaic pattern LinoleumTM-type vinyl floor sheeting containing asbestos in the backing (approximately 600 square feet). Refer to Table 1 and Figure 1 for detailed sample descriptions and ACM locations.
The previous Asbestos Survey report conducted by OHC Environmental Engineering, dated
September 29, 2014 identified asbestos in Red Duct Mastic and Sink Undercoating in addition iii to the flooring materials and mastics identified in this report. The Red Duct Mastic and Sink
Undercoating on the first floor should be assumed positive.
Recommendations
Recommendations resulting from the Baseline Asbestos Assessment Survey include the following:
Prior to disturbance activities, any confirmed and/or assumed ACM should be abated or provided with appropriate response actions. These materials are summarized above and within
Section 4.0 of this report.
Additionally, any other suspect material not included in this report should be treated as ACM, and should not be disturbed until sampled, tested, and proven otherwise.
1. INTRODUCTION
Thunderbird-HRP was retained by the Orlando VA Healthcare System to conduct a baseline asbestos assessment survey for the Lake Baldwin Orlando VA Medical Center, Building 500 – Main Clinic, located at 5201 Raymond Street, Orlando, FL 32083 (hereinafter referred to as “the Site”). This included inspecting suspect flooring, wall, ceiling, duct and piping systems located in all areas of Building 500 (crawl space, Mezzanine, and Floors 1 – 4). The specific locations included in the survey are detailed in Section 2.0.
1.1. Purpose
The Veterans Health Administration (VHA) Directive 7714 (Asbestos Management Program), dated April
6, 2017, calls for a survey following requirements of the Asbestos Hazard Emergency Response Act
(AHERA), known as a baseline survey. The purpose of the baseline survey is to identify, quantify, and sample suspect asbestos containing material (ACM) throughout the building. The survey was conducted in accordance with the Occupational Safety and Health Administration (OSHA) and the Environmental
Protection Agency (EPA) regulations. Non-suspect materials, which are defined as materials that are not expected to contain asbestos, were not sampled or analyzed as part of this survey.
1.2. Scope of Services
This baseline asbestos assessment survey was performed using general procedures and protocols appearing in EPA 40 CFR, part 763 (AHERA Protocols), and included the following general components: (1)
Site walkthrough; (2) Identification, classification, and bulk sample collection of suspect ACM; and (3) interpretation of laboratory analytical results. The scope of services provided by Thunderbird-HRP included the following tasks:
Review of applicable regulatory standards on asbestos and pre-demolition/renovation requirements;
Coordination with a NVLAP accredited laboratory for bulk sample analysis;
Walkthrough of all areas of interest with the Site representatives and review the scope of the project;
Identification, classification, quantification, delineation, and bulk sample collection of suspect ACM which is present within the building; and
Interpretation of laboratory PLM analytical reports.
No significant scope-of-work additions or modifications other than those noted below.
1.3. Project Limitations
The following areas were not inspected due to inaccessibility, or at the request of the site contact:
Roof / Roofing materials, Building Exterior
CT Scan Addition – Under Construction
Beneath concrete floors , D
Behind or inside concrete, masonry, cementitious, rigid, or CMU Walls or Parapets or CMU walls and cavities (loose fill insulation)
Confined Wall and floor utility chases, pipe chases and/or tunnels, inside/behind walls and wall cavities, pipe/utility trenches, etc vinyl/metal windows/casings (caulking & glazing)
Mirror Glue (in Restrooms)
Room 1720, 266 & 272, Clean Rooms – Above the ceiling and under the vinyl tile was not inspected.
If any additional materials other than those sampled and reported in this survey are identified within the building, they should be treated as asbestos-containing until proven otherwise via sampling and analysis in accordance with applicable regulations.
2. ASBESTOS BULK SAMPLING
The baseline survey for Building 500 was conducted on October 24th through October 28th, 2024, by
Thunderbird-HRP accredited asbestos inspectors. The survey included collecting representative samples of suspect asbestos-containing materials (ACMs), assessing their condition, and quantifying the materials and hazards present throughout the building.
2.1. Sampling Methodology
Thunderbird-HRP personnel systematically inspected the Site building as described above, and specified by the Site representatives. During the survey, Thunderbird-HRP classified each suspect ACM as one (1) of three (3) types:
Surfacing Materials - ACM sprayed or troweled on surfaces (walls, ceilings, structural members) for acoustical, decorative, or fireproofing purposes.
Thermal System Insulation (TSI) - Insulation used to inhibit heat transfer or prevent condensation on pipes, boilers, tanks, ducts, and various other components of hot and cold-water systems and heating, ventilation, and air conditioning (HVAC) systems. This includes pipe lagging, pipe wrap;
block, batt, and blanket insulation; cements and “muds”; and a variety of other products such as gaskets and ropes; or, Miscellaneous Material – Other, largely non-friable products and materials such as flooring, floor tile, ceiling tile, roofing felt, concrete pipe, outdoor siding, and fabrics.
Thunderbird-HRP inspectors touched all suspect materials to determine the extent to which the material, when dry, may be crumbled, pulverized, or reduced to powder by hand. During the survey, suspect materials identified by the inspector were also evaluated for overall condition, and each suspect material was assigned a condition of significantly damaged, damaged, or good overall condition for the locations the materials were observed. As defined by the US EPA under Asbestos Hazard Emergency Response Act
(AHERA), a material is significantly damaged if ten percent (10%) or greater of the material is evenly deteriorated, crumbling, hanging, or if the material contains over 25% localized damage. A damaged material is one that has less than 10% evenly distributed damage or less than 25% localized damage. A material in good condition is a material with no visible damage or deterioration or showing only very limited damage or deterioration.
Each suspect ACM was also classified into a homogeneous sampling area (HA). An HA is an area which is uniform in texture and appearance, was likely installed during the same period, and is unlikely to consist of more than one type or formulation of material. Thunderbird-HRP inspectors collected samples of each material from each HA, with multiple samples of a single material collected as appropriate. Samples were not collected from materials that were readily identifiable as non-asbestos containing (i.e., wood, fiberglass, foam, rubber, paper wraps, etc.) or previously confirmed to be asbestos containing (if applicable).
To avoid disturbing the material any more than necessary and to minimize any unnecessary release of potential asbestos fibers during collection, Thunderbird-HRP’s inspectors performed bulk sampling of suspect ACM in accordance with generally accepted procedures outlined in the US EPA document
"Guidance for Controlling Asbestos-Containing Materials in Buildings" (EPA-560/5-85-024, June 1985).
Each sample was collected using wet methods and placed in clean, sealable plastic bags and labeled with a unique sample identification number.
Bulk samples were submitted to Arrowhead Technologies, LLC, in Clearwater, Florida, a National Voluntary
Laboratory Accreditation Program accredited laboratory (NVLAP code 200703-0), for analysis of asbestos content by the Polarized Light Microscopy (PLM) method.
2.2. Non-Suspect Material(s)
Non-suspect materials, which are defined as materials that are not expected to contain asbestos, were not sampled or analyzed. Non-suspect materials typically include general building materials (e.g., glass, metals, brick, fiberglass, concrete, wood, cinderblock, and rubber). Please note that if a suspect material is later identified on-site and is not among those materials identified in this report, that material must be assumed to be asbestos containing until sampled and proven otherwise. Refer to Table 1 for Suspect ACM
Sampling Information.
3. SITE PROVIDED INFORMATION
During this assessment, Site contacts provided Thunderbird-HRP with access to individual areas of the building.
3.1. Previous Assessment Activities
Site contacts provided Thunderbird-HRP with the following ACM survey:

Homogeneous sampling areas identified in this table are based on information observed at the site. Material locations and total quantities for each suspect material were estimated at the discretion of the survey inspector(s) based on only observations during the survey. These should not be construed with total locations/quantities for the site building. Quantities and locations of underlying materials such as mastics should also be confirmed at the time of abatement due to difficulty with field confirmation as estimates are based on only those locations sampled during the inspection.
*Homogeneous areas 87-99, 122-126, and 130-141 are not included in this report as they pertain to other buildings on-site.
