About this file

This document is a Hazardous Energy Control Contractor Compliance Certification form for contractors working at the Portsmouth Naval Shipyard. The document requires contractors to certify understanding and compliance with 29 CFR Part 1915.89 and the Naval Sea Systems Command (NAVSEA) Occupational Safety, Health and Environmental Corporate Manual Chapter 250, specifically addressing Lockout/Tags-Plus (LOTP) procedures for hazardous energy control.

The document includes a warning that failure to comply with these requirements may result in serious injury or death, and that the Portsmouth Naval Shipyard will not assume liability for contractor responsibilities. Contractors must sign the document, confirming their understanding and agreement to these safety protocols, and email a completed copy to specified Navy personnel.
