About this file
This is a Past Performance Questionnaire for NASA Armstrong Flight Research Center's Request for Quote No. 80AFRC26Q0006, titled "Geometry Optimization and Sensing with Integration and Flight Test (GOSWIFT)."
The questionnaire is designed to collect feedback from current or previous customers regarding a contractor's past performance on completed or in-progress contracts. Responses must be submitted by 5:00 PM Pacific Daylight Time on June 2, 2026, to Jenni Schnarr (jennifer.schnarr@nasa.gov) and Tanjanica Jackson (tanjanica.s.jackson@nasa.gov). The questionnaire requests information on contract details including project title and contract number, total project value, whether the offeror served as prime contractor or subcontractor, contract type, and scope of work. Respondents must provide ratings across five evaluation categories—Quality, Schedule, Management, and an optional "Other" category, plus an Overall assessment—using a six-point scale: Unsatisfactory, Marginal, Satisfactory, Very Good, Exceptional, or N/A. Each rating requires a narrative explanation from the evaluator. The questionnaire also requests additional beneficial information, the evaluator's contact information, their role on the project, and the length of their involvement. Responses are treated as sensitive information and cannot be released to the offeror.
