Statement of Work

The Green Mountain National Forest (GMNF) has potable drinking water wells required to comply with both the Safe Drinking Water Act (Federal direction enforced by Vermont DEC) and Forest Service Directives. They are a mix of Public and Private Drinking Water systems and to comply, there are various water quality tests required. See Attachment 1 - Schedule of Items - 12444325Q0032 and Attachment 3 - GMNF Site Information for additional details.

The purpose of this contract is to find a vendor who will furnish water sample kits and who can provide a courier service to transport samples provided by the Forest Service, to a Lab Certified for Drinking Water Analysis by the Vermont Department of Health (Attachment 4 - Vermont Department of Health Lab Listing) capable of completing the required testing. The vendor will also be required to follow the Lab Analysis and Reporting Requirements as outlined below. This includes completing the Vermont DEC required electronic transmissions (timeline and format) for all Public Water System analysis results.

All costs shall be paid for by the vendor. The pricing provided on the Schedule of Items must be all inclusive of any and all costs associated with this project. This includes but is not limited to: procuring test kits, transporting samples, laboratory testing costs, completion of required Vermont DEC transmissions, etc… NOTE: It is not known exactly how many samples will be needed until samples are taken and analyzed. Therefore, the estimated quantities in Attachment 1 - Schedule of Items - 12444325Q0032 are estimated only. The estimated quantities may only be exceeded with approval from the contracting officer in writing.

Sampling kit delivery:

The Forest Service will coordinate with the contractor to have the sample kits sent to the sites when they need testing. Refer to Attachment 3 - GMNF Site Information for shipping addresses and sampling schedules. Base Year and Option Years vary by sites, see Attachment 3 - GMNF Site Information for additional information. Repeat samples, if necessary, will be shipped after discussion with the COR or sampler. Also, incidental to providing samples, furnish and deliver 4 gallons of 12.5% sodium hypochlorite bleach to the Manchester Ranger Station each April.

Forest Service Sample Collection and Delivery:

The Forest Service will collect and deliver all samples to a courier provided by the contractor or laboratory. The courier pick-up shall be within 20 miles of where the sample kits were shipped/delivered. Forest Service samplers are responsible for delivering them within viable sample timeline, on ice (per sampling instructions), and with the complete and correct paperwork.

Vendor is required to maintain viability of sample while transporting to laboratory for testing. If needed, the vendor shall provide a cooler for the samples and pay for any shipping fees.

Lab Analysis and Reporting: Requirements by category as described below and in accordance with the Vermont Water Supply Rule.

1. Certification

· Lab must be certified in Vermont to test drinking water.

2. Analysis:

· Analyze all viable samples received.

· Estimated quantities on Attachment 1 - Schedule of Items - 12444325Q0032 may not be exceeded without written approval from the contracting officer.

· If a sample received is deemed non-viable due to spillage, age, or improper paperwork, notify the sampler immediately so the sample may be retaken.

3. Notification and Reporting

· If a coliform sample is safe, no immediate notification required.

· If a sample is Coliform +/E. coli -, notify the sampler by phone the same day; leaving a voicemail is acceptable.

· If a sample is Coliform +/E. coli +, notify the sampler by phone immediately.

· If you do not reach the sampler, leave a voicemail -AND- notify the COR by phone immediately. If you do not reach the COR, leave a voicemail there as well.

· If any other results exceed set MCL limit, notify COR by phone immediately. Otherwise, no immediate notification is required.

4. Reporting Results

· Comply with Vermont DEC required electronic transmission (timeline and format) for all Public Water System analysis results.

· Produce reports for Private Water Systems including, but not limited to: System Name, Unique Sample Identification Number, Date and Time Collected, Name of Sampler, Location of Sample, and Result.

· Email all results (public and private) to the sampler and the COR within 5 business days of analysis completion.
