Overview & Instructions

Gloves Quote Sheet Instructions

Overview Suppliers will use this workbook to quote gloves and provide item details in response to this solicitation.

Workbook tabs This quote sheet workbook contains 3 separate tabs:

Overview & Instructions Gloves Quoted Supplies- Quoted Gloves for the VA and item details shall be entered and priced here Data Field For Reference - Definition and examples of data fields to complete

Glove Quoted Supplies – Enter Data on this tab Supplier information shall be filled out completely at the top, ensuring business designation is accurately captured.

Suppliers should add glove details and price quotes in this tab for each quoted glove. Suppliers should enter each quoted glove on a separate line (i.e. each glove type and size should be on its own line). Please include the glove size in the description field.

The first two columns ("Index" and "For Internal Validation Purposes (set to 'No' by default)") are locked as they populate automatically based on user input in the other columns. Users cannot change these values.

For each glove quoted, manually complete all the GREEN fields; the copy paste data function is limited by the quote sheet’s validation macros. BLUE fields are optional, but are highly encouraged for completion if the information is known.

For the "OEM UOM Description" and "Smallest UOM Available for Facility Purchase" fields, select an option from the drop down list.

This quote sheet has built in validation that will alert the user when they enter data in the wrong format, e.g. filling in a field as text when it should be a date or number. The RED fields indicate missing information required for submission; RED text will change to GREEN when corrected.

Blank and NULL cells are counted as errors and are summarized at the top of the Gloves Quoted Supplies sheet. Quote sheets with errors will not be accepted. Do not submit the files until there are no errors identified by the sheet.

Key Steps to being able to edit the Quoted Supplies worksheet

1) Ensure to save and work from a local version of the file

2) Click on 'Enable Editing' if the warning pops-up

3) Click on 'Enable Content' to enable the built in validation macros of the workbook. The validation macros will assist you in filling out the Quoted Supplies tab

23NSNThe NSN is a 13-digit numeric code that identifies 'standardized material items of supply' recognized by DoD and NATO. The first 4 digits contain the Federal Supply Classification Group (FSCG) or Product Service Code (PSC), followed by the 9-digit National Item Identification Number (NIIN). The NIIN consists of a 2-digit National Codification Bureau number designating the country of final manufacture or the country that assigned the number. NCB Codes of 00 or 01 indicate the USA. An additional 7-digit (xxx-xxxx) non significant number follows. The number shall be arranged as follows: 9999-00-999-9999.StringSupplier6510-01-652-4867

FDA is establishing a unique device identification system to adequately identify medical devices through their distribution and use. When fully implemented, the label of most devices will include a unique device identifier (UDI) in human- and machine-readable form. Device labelers must also submit certain information about each device to FDA’s Global Unique Device Identification Database (GUDID).

27Global Trade Item Number (GTIN)GTIN describes a family of GS1 (EAN.UCC) global data structures that identify trade items in a machine readable form (or Bar code). GS1 is comprised of partners from the International and Uniform Code Council (formerly - EAN - European Article Number). For North American companies, the UPC is an existing form of the GTIN, but additional structures with up to a 14-digit number can be used to identify trade items at various packaging levels such as box or case.StringSupplier20705031203522
28UNSPSCThe United Nations Standard Products and Services Code® (UNSPSC®). The UNSPSC was jointly developed by the United Nations Development Program (UNDP) and Dun & Bradstreet Corporation (D & B) in 1998. UNSPSC is an open, global, multi-sector standard for classification of products and services, UNSPSC encompasses a Hierarchical 4-level tree structure: Segment, Family, Class and Commodity. The UNSPSC code is eight digits, the first two correspond to the segment code, the second two to the family, the third two to the class and the final two to the commodity. You may browse and download the current version of the code at no cost at the below website.
