PERFORMANCE WORK STATEMENT (PWS)

FOR

GARNETT REMOVAL FROM THE

CUTTING TABLE

Section One

1.0 Scope:

The Contractor shall provide all equipment and personnel necessary to clean the Cutting Table (OMAX 55100 JetMachining Center) located at the 307th Metal Shop on Barksdale Air Force Base (AFB). This service includes the removal and disposal of all collected water, sand, sludge, and debris from the cutting table. Agitation or cleaning may be required to remove solids adhered to the system chambers and components. All materials shall be disposed of at an approved disposal facility in accordance with all applicable regulations. See Appendix 1 for reference photographs.

1.1 Specific Requirements:

The Contractor shall:

1. Furnish all parts, labor, tools, materials, transportation, facilities, and supervision required to clean the cutting table at Barksdale AFB, Louisiana.

2. Remove and transport all collected materials to an authorized disposal location, complying with all applicable Federal, State, and local regulations. The Contractor shall possess all appropriate licenses and have made all required notifications to the relevant regulatory authorities (e.g., LDEQ, DOTD, EPA) for transporting, handling, storing, and processing the materials. The Contractor must be in "good standing" with these agencies prior to commencing work.

3. Perform work on an as-needed basis, typically twice per year. The 307th Point of Contact (POC) will coordinate the scheduling of cleaning services.

4. Provide a copy of the manifest/bill of laiding after each disposal, indicating the disposal facility used and the amount of material disposed of. This documentation shall be submitted electronically to 307th POC or mailed to: 334 Davis Ave W., Suite 208, Barksdale AFB LA, 71110.

5. Submit an invoice to the 307th POC after completion of each work order via WideArea Workflow.

6. Accept payment at the contract unit price as full compensation for all labor, equipment, and materials required to complete the work. Payment will be made using a government purchase card.

1.2 Hours of Operation:

The Contractor shall perform work Monday through Friday, from 7:00 AM to 5:00 PM. Any work required outside these hours must be pre-approved by the Contracting Officer (CO) and coordinated with the Government Subject Matter Expert (SME), who must be present during such work. These hours exclude Federal Holidays, base closures due to inclement weather, and furlough days, as applicable.

1.3 Contractor Personnel:

• Contract Manager (CM): Within five (5) business days of contract award, the Contractor shall designate a CM and any alternate(s) in writing to the CO and SME. The notification shall include telephone numbers and email addresses where the CM and alternate(s) can be reached at any

Attachment 2 time. The CM or alternate shall have full authority to act on behalf of the Contractor regarding all matters related to this contract at the 307 MXS on Barksdale AFB. The CM or alternate shall be available to meet with Government personnel to discuss any issues. The CM shall be fluent in reading, writing, speaking, and understanding English.

• Identification: Contractor personnel shall identify themselves as contractor personnel to Government personnel and shall wear visible identification badges while performing work for the 307 MXS on Barksdale AFB.

• Employee Listing: The Contractor shall provide an initial list of employee names and position titles to the SME within five (5) business days of contract award. An updated list shall be provided within ten (10) business days whenever employee status or positions change.

• Employee Restrictions: The Contractor shall not employ individuals identified as potential threats to the health, safety, security, well-being, or operational mission of the installation. The Contractor shall not employ any US Government employee if such employment would create a conflict of interest. Employment of Department of the Air Force (AF) employees, either military or civilian, requires approval according to DoD 5500.7-R Joint Ethics Regulations (JER) and compliance with Air Force Instruction (AFI) 64-106, Air Force Industrial Labor Relations Activities . Off-duty military personnel providing surveillance on any contracts/sub-contracts awarded to the Contractor shall not be employed.

• Conduct and Attire: All contract personnel shall wear professional and suitable attire and conduct themselves in a professional and courteous manner.

• Workforce Management: The selection, assignment, reassignment, transfer, supervision, management, and control of contractor personnel are the responsibility of the Contractor.

However, the Contractor shall comply with the requirements of this PWS and Barksdale AFB regulations regarding employee conduct. The Contractor shall employ and maintain a technically trained and experienced workforce.

1.4 Environmental Protection & Safety:

The Contractor shall comply with all Federal, State, and local regulations regarding the handling, transportation, and disposal of collected materials. This includes possessing the appropriate licenses and having made the required notifications to the relevant regulatory authorities (e.g., LDEQ, DOTD, EPA).

1.4.1 Collected materials from the cutting table shall be handled, transported, and disposed of in strict accordance with the requirements found in 40 CFR.

1.4.2 A completed manifest/bill of lading must be provided by the Contractor for each shipment of garnett, listing the amount of material removed, the date of custody transfer, and information regarding the disposal location.

1.4.3 Contractor operations must comply with all state and federal laws and regulations. The contractor is responsible for obtaining all required applications, permits, or approvals.

1.4.4 Contractors shall comply with the Barksdale Air Force Base (BAFB) Supplement to Air Force Instruction AFMAN32-7002, "Hazardous Materials Management "; Barksdale AFB Hazardous Waste

Management Plan (BAFB HWMP); procedures per 307 Maintenance Operations Squadron Supplement to AFMAN32-7002, "Hazardous Material Management "; and OC-ALCI 32-101 "Air Quality Record Keeping Procedure ". Publications are available at www.e-publishing.af.mil.

1.5 Hazardous Material (HAZMAT):

If no HAZMAT will be brought onto the base or used during the performance of this contract, the contractor shall provide a written certification to the SME stating "No HAZMAT will be brought on base" or "No HAZMAT Usage" within ten (10) days after award. Upon receipt of this certification, the requirements of this section no longer apply. Penalties or disciplinary action may be assessed for violations of this certification.

1.6 Off-Site Discharges:

For discharges outside of Barksdale AFB, the Contractor shall ensure wastewater discharges containing pollutants resulting from contracted services are directed to a state or federal permitted treatment facility, such as a municipal Publicly Owned Treatment Works (POTW). Within ten (10) calendar days of contract award, the Contractor shall provide the SME with written confirmation from the POTW stating that the facility is permitted for all pollutants that the Contractor may discharge.

1.7 Safety and Health on Government Installations:

While performing work on a Government installation, the Contractor shall comply with all federal, state, and local regulations, and AFI 91-203, "Air Force Consolidated Occupational Safety Instruction ," as established by this contract and Appendix C. The Contractor shall notify the CO, SME, and Government Contract Administrator (CA) within one (1) business day of any injuries to contractor personnel or damage to Government property or equipment during the execution of the contract.

1.8 Emergency Procedures:

Contractor personnel shall follow the direction of Government personnel during emergency procedures (e.g., fire, tornado, active shooter, bomb threats). The Contractor is responsible for providing guidance and establishing procedures for its employees to respond to emergencies and for accounting for employees following an evacuation. The Contractor shall participate in all fire and tornado drills at no additional cost to the Government.

2.0 GOVERNMENT FURNISHED PROPERTY AND SERVICES.

2.1.1 Fire Emergency and Routine Services: The Government will provide general on-base fire protection services. The Contractor shall comply with fire prevention rules and regulations enforced by the BAFB Fire Prevention Branch. Call 911 for emergencies; for routine fire prevention matters, call 318- 456-2642.

2.1.2 Workspace: The Government will provide access to the equipment and adequate workspace around the equipment.
