About this file

This document is a DBIDS (Defense Biometric Identification System) Card/Pass application form for Columbus Air Force Base in Mississippi. The form collects personal information from individuals requesting base access, including name, Social Security number, date of birth, driver's license information, and company affiliation. Applicants must authorize the release of their information to Columbus AFB for background checks, including Wants/Warrants checks, National Criminal Information Center (NCIC) searches, and Security Forces Management Information System (SFMIS) reviews to determine access eligibility. The form requires two forms of identification (one with a picture) to be presented at the Visitor Center, and applicants must acknowledge that badges are government property and must be returned upon termination of base access eligibility.

The 14th Security Forces (14 SFS/S5B) completes the background check verification section, approving or disapproving access and issuing the badge with an expiration date not to exceed one year. Request forms are valid for 60 days from the request date and must be reaccomplished if not processed within that timeframe. The authorization is based on 10 U.S.C. 8013, DoD Directive 7730.47, Air Force Instruction 31-203, and the Brady Handgun Violence Prevention Act. This form serves as a supporting document for the FY 26 Columbus Laundry and Linen Recompete contract and would be required for contractor personnel needing base access to perform laundry and linen services at Columbus Air Force Base.
