About this file

This is a Conflict of Interest Affidavit and Disclosure form for the Jefferson Patterson Park and Museum (JPPM) in Maryland, requiring contractors to certify compliance with conflict of interest regulations prior to contract award. The document serves as a mandatory disclosure requirement for bidders on the septic tank pump-out project, which involves services at nine separate facilities across the JPPM campus located along Mackall Road. The scope of work requires the selected contractor to pump out septic tanks ranging from 1,000 to 10,000 gallons across multiple buildings including the Beverly House, King's Reach, Breckenridge Center, Visitor's Center, Pavilion, MAC Lab, Gardener's Cottage, and Patterson Estate. The contractor must complete all work within 60 calendar days of receiving authorization to proceed and must maintain all applicable national, state, and local licenses or certifications, with a designated Master License or Certification holder responsible for all services performed.

The affidavit requires bidders to disclose any potential conflicts of interest, financial interests, or relationships that could compromise their objectivity or create improper influence in the contracting process. All contractors and subcontractors must be licensed in Maryland or appropriately certified for the nature of work being performed. The disclosure form ensures compliance with state procurement regulations and protects the integrity of the competitive bidding process, requiring full transparency regarding any circumstances that could affect the contractor's ability to perform the work impartially and in accordance with applicable laws, regulations, and codes.
