About this file
This is a Worker Protection and Investment Certification Form (BOP-2201) required by the Commonwealth of Pennsylvania for all contractors and grantees, specifically applicable to the Pennsylvania Liquor Control Board (PLCB) background check contract opportunity. The form mandates that contractors certify compliance with Pennsylvania's Unemployment Compensation Law, Workers' Compensation Law, and all applicable state labor and workforce safety laws, including thirteen specific statutes covering areas such as workplace misclassification, child labor, minimum wage, prevailing wage, equal pay, and construction industry verification. The certification must be executed by an authorized official of the contractor or grantee and requires immediate reporting of any changes in compliance status to the purchasing agency.
Pennsylvania law imposes significant penalties for false certifications, including contract termination and a three-year prohibition on bidding for state contracts under the debarment and suspension provisions of 62 Pa. C.S. § 531. The certification is subject to the penalties outlined in 18 Pa. C.S. § 4904 regarding unsworn falsification to authorities. No pricing terms, set-asides, incumbent information, or contract-specific financial details are addressed in this attachment; the form serves solely as a compliance verification document required before contract award or performance.
