About this file
This is an Administrative Requirements document for a Torula Dried Yeast procurement (BU 8570 Event ID 0000033857) from the California Department of Food and Agriculture (CDFA) for delivery to their Hawaii Fruit Fly Rearing Facility in Waimanalo, Hawaii. The delivery must be completed within 365 calendar days after the Purchase Order date, with orders placed on an "as needed" basis throughout the contract period. Each shipment requires a certificate of analysis showing chemical composition and microbial content, and primary shipping bags must be labeled with product details including shelf-life date and batch number. The State allows partial deliveries and payments upon inspection and acceptance.
The contract includes provisions for unanticipated tasks with an additional 10% of the total contract/purchase order value reserved for State-required and approved additional work. Any changes or additional work must be approved through a Work Authorization Form (Attachment 5), which must include detailed work statements, time schedules, and itemized costs. No Purchase Order Change or Amendment is required for approved Work Authorization Forms. The contractor is not entitled to any portion of the additional 10% unless the extra work is documented and approved by CDFA.

Attachment 2
Administrative Requirements
1. DELIVERY
Delivery is to be completed in full within three hundred and sixty-five (365) calendar days after Purchase Order date.
Delivery address: CDFA Hawaii Fruit Fly Rearing Facility, 41-650 Waikupanaha Street, Waimanalo, Hawaii, 96795.
Delivery timelines can be changed if both, the state, and the contractor agree.
Approval for extensions will be done using a Work Authorization Form (Attachment 5). The Contractor will submit a Work Authorization Form to California Department of Food and Agriculture (CDFA) for approval and after CDFA‘s approval, can proceed with the approved changes. The approved Work Authorization Form will thereby be incorporated to the contract. A Purchase Order Change or amendment will not be required.
A Order and Delivery Requirements
Order will be on an “as needed” basis throughout the contract period.
B A certificate of analysis shall be included with each shipment. This certificate must show, at a minimum, the chemical composition and microbial content as stated in the bid specifications.
C The exterior of each primary shipping bag shall be labeled with the product's name, brand name, net weight, unit count, and shall include a clearly visible end of shelf-life date, and a manufacturers lot batch number.
2. PARTIAL DELIVERIES
The State will allow the invoicing and payment of each unit after the contractor completes delivery and inspection of each unit and is accepted by the California Department of Food and Agriculture.
3. UNANTICIPATED TASKS
In the event that additional work must be performed which was wholly unanticipated and is not specified in the Administrative Requirements, but which in the opinion of the State, is necessary to the successful accomplishment of the general scope of work outlined, the procedures outlined in this section will be employed. However, no such changes may commence without the prior approval of the California Department of Food and Agriculture (CDFA) and via an approved Work Authorization Form (Attachment 5). After CDFA‘s approval, the Contractor

Attachment 2 can proceed with the approved changes, and will be paid from this reserved allowance.
For the purpose of this procurement, the State shall allow an amount equal to 10% of the total contract/purchase order (PO), to be added to the contract/purchase order. This is the maximum amount that will be available for State required and approved additional work, etc. after Purchase Order award.
The contractor is not entitled to any portion of the additional 10% unless the extra labor, tasks, etc. are documented and approved by the CDFA. A Purchase Order Change or Amendment will not be needed.
Each Work Authorization Form shall consist of a detailed statement of the work to be performed, an estimated time schedule for the performance of the work, and the Contractor’s itemized and total one-time and on-going costs, or cost reductions, of the Work Authorization. Prior to commencing work, the Work Authorization Form must be signed by the Contractor’s designated representative and by the CDFA‘s or designated representative. The approved Work Authorization Form will thereby be included in the contract. A Purchase Order Change or Amendment will not be needed.
4. WORK AUTHORIZATION FORM
If the California Department of Food and Agriculture requires additional work, such as adding transactions or features, that is unanticipated and is not specified in the Contract but is, in the opinion of both parties, necessary to the successful accomplishment of the general scope of work outlined, the Work Authorization Form (Attachment 5), will be used to obtain the approval. The approved Work Authorization will thereby be included in the contract. A Purchase Order Change or Amendment will not be needed.
