VA SORCC Statement of Work 692-25-104 Abate B219 Ceilings at select rooms
A. GENERAL
A.1 GOAL:
Contractor shall provide White City, Oregon, VA Southern Oregon Rehabilitation Center and Clinics (VA SORCC) with asbestos containing material (ACM) abatement and construction services in building 219.
A.2 LOCATION OF SERVICES:
VA SORCC

VA site BUILDING 219.
A.3 SCOPE –
There are 4 rooms (approx.. 416 sq.
ft.) that need the ACT ceiling removed and replaced along with 6 lights, and 2 vent fans. Above the ACT ceiling on the ceiling structure contain ACM tiles that need abatement. The room numbers are 105, 106, 108 and 109 in building
219. This project must be done in two different phases. Rooms 105 and 106 will be one phase and then rooms 108 and 109 will be another phase. This always allows Canteen Services access to their storerooms
104 and room 107 during construction. The removal of items will need to follow ACM removal and ICRA procedures to prevent contamination of area and people outside of ICRA barriers. Rooms 108 and 109 materials must be removed through the window.
In the image on the right, it shows the ACM asbestos tile connected to the existing structure, and the ACT ceiling that is beneath the asbestos tile.
All of this will need to be removed and replaced. The drop-down ceiling tile will need to be replaced with tile in accordance with the specifications. The vent fans will be replaced. All lights will need to be changed out to LED in accordance with the new federal regulations.
B. CONTRACTOR
B.1 FURNISHED PROPERTY OR SERVICES:
Contractor to furnish labor, personnel, materials, and equipment to perform Asbestos abatement on the ceiling of four rooms (approx. 416 sq. ft.) in building 219.
B.2 CONTRACTOR DELIVERABLES
Before work can begin a detailed action plan with a detailed timeline must be submitted and accepted at a minimum of 1 week before work starts.
Contractor shall provide a Report for all items completed. This includes the waste report, and all construction completed. As a minimum the record documents must provide:
a) The name of the company.
b) Name and signature of Qualified Professional rendering the service.
c) Date of when the services are rendered.
d) Small Description of what service was rendered.
d) Any follow-up required.
Coordination Drawings will be required at the end of construction.
C.6 VA SORCC Business Hours:
Work will need to be scheduled with 1 week notice for all work that is outside normal VA business hours 7 AM to 4:30 PM. Weekends and after hour work is acceptable if it required. No work will be allowed on federally recognized holidays.
C.7 QUALIFICATIONS:
a) All personnel that will be on the job will need to be submitted as key personnel or SF1413 for subcontractors.
b) Must have experience with ACM abatement.
C.8 LAWS & ORDINANCES
The Contractor shall comply with all applicable laws, ordinances, and regulations (Federal, state, city or otherwise).
Period of performance is 90 days from Notice to Proceed.
