About this file
This document is a Business Associate Application for a company seeking to work with the Veterans Health Administration (VHA), specifically focused on privacy and security requirements for handling Protected Health Information (PHI). The comprehensive 11-page form requires detailed information about the company's data handling practices, including how PHI is accessed, created, transmitted, maintained, stored, and destroyed. Key sections cover the company's workforce, contact information for privacy and security officers, number of employees with data access, locations where data is maintained, electronic and hard copy access methods, and compliance with HIPAA regulations.
The application requires companies to disclose their functions performed for VHA, the types of VHA program offices or medical centers they serve, data breach history, subcontractor relationships, and policies for protecting patient information. Companies must provide signatures from their company representative, privacy/compliance officer, and information security officer, affirming the accuracy of their responses. The form serves as a thorough screening mechanism to ensure potential business associates understand and can comply with VHA's strict data privacy and security requirements before engaging in contracts involving veteran health information.
