About this file
This is a Past Performance Questionnaire (PPQ) template used by the U.S. Department of the Interior, Bureau of Reclamation, Boulder City, Nevada to collect evaluations of contractor performance on completed projects. The form serves as an appendix to federal contract solicitations and does not address specific products or services required.
The questionnaire collects detailed information about past contract performance through evaluator ratings on sixteen criteria, including firm-client relationships, subcontractor management, corporate management and integrity, work quality, quality control, schedule adherence, cost control, labor standards compliance, and safety standards compliance. Evaluators assign ratings using a five-point scale: "O" (Outstanding—greatly exceeded requirements), "A" (Above Average—exceeded requirements), "S" (Satisfactory—met requirements), "M" (Marginal—met minimum requirements with some unsatisfactory aspects), and "U" (Unacceptable—poor performance). The form requires evaluators to identify the firm being evaluated, contract number, project title, award and completion dates, initial and final contract amounts, and the evaluator's contact information and relationship to the project. Yes/No questions address whether cure notices, show cause letters, or terminations were issued; whether the evaluator would award future contracts; customer satisfaction with deliverables; and whether the firm had opportunity to respond to negative comments. Completed questionnaires should be submitted to the Bureau of Reclamation via email to habellon@usbr.gov.
