GENERAL REQUIREMENTS
PART 1 - GENERAL
1.1 SAFETY REQUIREMENTS
A. Refer to section 01 35 26, SAFETY REQUIREMENTS for safety and infection control requirements.
1.2 GENERAL INTENTION
A. Contractor shall completely prepare site for building operations, including demolition and removal of existing structures, and furnish labor as required by drawings and specifications.
B. Visits to the site by Bidders may be made only by appointment with the Contracting Officer’s Representative.
C. Offices of West Point Engineers, as Architect-Engineers, will render certain technical services during construction. Such services shall be considered as advisory to the Government and shall not be construed as expressing or implying a contractual act of the Government without affirmations by Contracting Officer or his duly authorized representative.
D. Before placement and installation of work subject to tests by testing laboratory retained by Department of Veterans Affairs, the Contractor shall notify the COR in sufficient time to enable testing laboratory personnel to be present at the site in time for proper taking and testing of specimens and field inspection. Such prior notice shall be not less than three workdays unless otherwise designated by the COR.
E. Prime Contractor Personnel Minimum Requirements:
1. As Required in other parts of the Project Manual and Contract Requirements the following are minimum requirements:
a. The Superintendent shall be an employee of the Prime Contractor with authority to act as the contractor, responsible for the day-to-day operations at the construction site, planning, quality control, verification of delivery of approved material and equipment, subcontractor coordination responsibilities and execution of the complete project as per the contract documents.
1) Superintendent shall have no hands-on construction interaction. In addition, the superintendent shall provide proof to the COR regarding completion of 30 hour OSHA.
2) Superintendent assigned to this project shall not be assigned to any other project and shall only have duties on one project at a time.
3) The Superintendent shall sign-in at the beginning of each day of work and Sign-out at the end of the work day at the Office of Engineering Service. No work shall be performed by any contractors or subcontractors without the presence of the superintendent at the actual work site.
4) The Superintendent shall be responsible for submitting daily work logs to the COR at the end of every work day.
b. Site Safety and Health Officer (SSHO) shall be a separate qualified individual from the Prime Contractor’s Superintendent or CQC System Manager with duties only as the SSHO. See 01 35 26 SAFETY REQUIREMENTS for more information on roles and responsibilities for General Contractor’s staff.
c. CQC System Manager shall be a separate qualified individual from the Prime Contractor’s Superintendent or Site Safety and Health Officer (SSHO) with duties only as the CQC System Manager. See Section 01 45 00 Quality Control for more information on roles and responsibilities for CQC Personnel.

F. Excavation Permit
1. The contractor shall complete the VA dig permit with the COR and submit the Dig Safe notification prior to start of any excavation work. See 01 35 26 SAFETY REQUIREMENTS for additional information. Excavation Permit requests to be submitted two (2) weeks prior to planned start of excavation work.
G. Electrical Work Permit
1. The contractor shall complete the VA Electrical Work Permit with the COR and submit to the COR a minimum of 72 hours prior to start of electrical work within the facility.
Electrical work permits shall be submitted and completed one (1) week prior to planned start of electrical work.
H. Welding and Cutting
1. A Hot Work Permit is required. See Section 01 35 26, SAFETY REQUIREMENTS for requirements. Hot work permit to be submitted and coordinated a minimum of 24 hours prior to planned start of hot work. Work can not commence prior to inspection of space by permit issuer. Contractor to adhere to current VAME requirements.
I. Penetration Permits, Confined Space Permits, Interim Life Safety Measures, Activity Hazard Analysis (AHA’s)
1. Confined Space Permits shall be required for (but are not limited to): Manholes, boilers, connecting tunnels, ductwork, and crawl spaces. Request for confined space permits should occur two (2) weeks prior to when access is needed. Project COR and Safety Officer must be notified prior to entry into confined space to allow for required inspections. Contractor follow VAME confined space permitting process.
2. Penetration Permit requests should be submitted two (2) weeks prior to planned start of penetration work.
3. Interim Life Safety Measures shall be coordinated within 30 days of NTP, or two (2) weeks prior to associated closure (which occurs first). Notification of implementation of ILSM shall be provided to the COR two (2) weeks prior to planned closures.
4. AHA’s shall be submitted within 30 days of Notice to Proceed, or two weeks prior to associated work activity (whichever occurs first).
J. Contract Work Hours
1. Normal working hours are 7:00 am to 3:30 pm. Contractors who wish to work other than
“normal working hours” for work to be done during those hours must submit a written request to the Contracting Officer for approval. The request must state that the Contractor is requesting to work other than normal working hours at no additional expense to the Government. Such requests shall be coordinated with the COR and must be submitted to the COR two (2) weeks in advance of planned work.
2. For certain aspects of work the Contractor shall be required to work off duty hours to perform that part of the work, for continued operations of the Medical Center. After duty hours work shall be coordinated with and approved by the COR.
a. Noisy demolition work, work at building entrances, electrical or other utility work necessary to pass within, above, traverse hallway corridors, temporary shutdowns of utilities for tie-in.
b. After hours work that is required in order to not impact on-going facility operations shall be provided by the Contractor at no additional cost to the Government.
K. Contractor shall provide Submittal Exchange®, Autodesk Build® and/or ProCore Project Site dedicated for this project, or approved equal (substitutions must be submitted with Contractor Bid), for the duration of the contract. Contractor will provide VA access to the site. Contractor will keep this site current and will use it for all submittals, RFIs, change documents, Record Drawing, electronic file management, and communication logs for the duration of the contract.
L. Contractor shall provide Infection Control Plan as required by Memorandum 111-40, Subj: VHA Directive 7715 Safety and Health During Construction dated April 6, 2017. Submit to COR for approval prior to the start of work.

M. Prior to commencing work, general contractor shall provide proof that an OSHA designated “competent person” (CP) (29 CFR 1926.20(b)(2) will maintain a presence at the work site whenever the general or subcontractors are present. CP will have authority letter.
N. Per VA Directive 1085, smoking, vaping and smokeless tobacco are prohibited on the grounds of VA facilities, including in vehicles. This Directive applies to all Prime Contractor and Subcontractor employees.
O. Contractor’s work shall fully comply with VHA Directive 1061 “Prevention of Health-Care Associated Legionella Disease and Scald Injury from Water Systems”, dated February 16, 2021. Contractor’s work activities, as well as submitted and installed products, must meet the requirements of this directive.
P. Prior to commencing work onsite, and each year thereafter, all contractor and subcontractor personnel shall provide proof of receipt of seasonal influenza vaccination in compliance with VHA Direction 1013.
Q. Prior to commencing work onsite, all contractor and subcontractor personnel shall provide proof of receipt of required COVID vaccinations and boosters in compliance with current VHA Directives.
R. Prior to commencing work onsite, all contractor and subcontractor personal shall provide proof of receipt of required Tuberculosis testing in compliance with VHA Directive 1131(5), dated June 4th, 2021.
S. Pricing Proposal transparency requirements:
1. This project requires pricing proposals to include breakdown pricing for the General
Contractor and all subcontractors.
2. All price proposals, shall including subcontractor pricing must include a breakdown of pricing per the format included with this Project Manual. Third-party inspections must be included as a line item in the price proposal.
T. A commissioning plan and commissioning Manager resume shall be submitted with contractor bids. The commissioning Manager (CxM) shall be a direct employee of the General Contractor and coordinate with the Commissioning Agent (CxA)
1. The CxM shall have been involved in a least three (3) comparable projects that have been completed within the previous five (5) years.
2. The proposed CxM should be certified by at least one of the following agencies:
a. National Environmental Balancing Bureau (NEBB)
b. AABC Commissioning Group (AGC)
c. Building Commissioning Association (BCxA)
3. Commissioning Management Services will be provided in accordance with the Department of Veterans Affairs Commissioning Process Manual. Prospective proposers shall review the Whole Building Commissioning Process Manual available on the VA Technical Information Library at http://www.cfm.va.gov/til/.

Submittal & Shop Drawing Review
Review all pertinent project shop drawings necessary to support the Commissioning Process. Review of the shop drawings is for the purpose of developing appropriate Pre- Functional Checklists and Systems Functional Performance Test Procedures. Submittals & Shop drawings shall be reviewed for commissionability, maintainability and for compliance to the Construction Documents.
Note any issues identified in the Shop Drawing Review that might compromise the final commissioned system on the ‘Commissioning Review Log’ and submit comment to the Design Team and the RE contemporaneously with the A/E review for resolution.
Comments shall be submitted in accordance with the contractual submittal review time.
Comments shall be provided to the Design Team no less than 7 days prior to the Submittal Review Due Date provided that the Cx Agent has no less than 7 days to review the submittals.

Commissioning Construction Observations
During the course of construction, the Commissioning Agent will visit the site to observe the progress of construction with respect to the systems being commissioned.
The focus of the observations will to verify that systems being installed comply with the Construction Documents and can be successfully configured, operated and maintained.
In addition, site observations visits will be scheduled to include
● Witnessing factory testing and/or contractor equipment/system start up activities;

Commissioning Team Meetings
The Commissioning Agent will hold regular commissioning team meetings to review progress of the commissioning effort and reinforce individual responsibilities. The team will review schedule, construction sequence, and completed work (PFC, PFT and FPT’s) and outstanding issues on the Commissioning Issues Log.
Commissioning Team Meetings may be held at the jobsite or by teleconference depending upon the status of construction. While it is imperative that many of the Commissioning Team Meetings be held in person, during early construction phases, some meetings may be more cost effectively conducted remotely.
Scheduling and location of meetings shall occur by consultation between the Commissioning Agent and the VA-RE.
The Prime Contractor’s scheduler should attend approximately every other Commissioning Team Meeting to review the master schedule and any changes that will affect commissioning activities.

Develop Final Pre- Functional Checklists (PFC’s)
The Commissioning Agent will prepare Pre- Final Pre Functional Checklists (PFC) for the project and distribute to all Cx Team members for review and comment. The Commissioning Agent will incorporate review comments and prepare Final PFCs.
An executed PFC submitted by the Prime Contractor indicates the system and its related equipment is ready for Systems Functional Performance Testing.
The requirements for Pre Functional Checklists shall be coordinated with the startup requirements specified for each commissioned system. The PFC’s shall be constructed to leverage the specified contractor and vendor start activities to avoid unnecessary duplication.

TAB Verification The Commissioning Agent shall work with the TAB Contractor to verify that all Testing and Balancing work is conducted in strict accordance with the specified Procedural Standards for Testing, Adjusting and Balancing by either NEBB or AABC. The Commissioning Agent shall execute the following tasks:
● Review the TAB Agenda provided by the TAB Contractor to verify that it is complete, thorough and in compliance with the specification and Procedural Standards requirements.
● On the project site, the CxA shall review the calibration certificates for the instrumentation being used by the TAB Contractor during the execution of TAB activities to confirm that the instrumentation meets the requirements of the spec and the appropriate Procedural Standard.
● The CxA shall periodically visit the project site and review the TAB documentation for method, reasonable values, and compliance with the specification, Procedural Standard and TAB Agenda.
● Upon completion of the TAB activities, the CxA shall visit the project site with the TAB Contractor and the RE. The TAB Contractor shall demonstrate readings selected by the CxA that demonstrate that the system performance is consistent with the submitted values on the TAB report.
Readings shall include Air Handling Unit total flows, static pressure control values, select outlet flows, building, and space pressures selected by the CxA as critical system operating values.

Systems Functional Performance Testing
The Commissioning Agent will oversee, facilitate, and document all FPT testing.
Execution of FPT’s shall be executed in accordance with the Functional Testing Procedures published by the Commissioning Agent. All systems tested in accordance with the FPT’s shall be operated by the contractors in accordance with the approved FPT.
Contractors shall retain responsibility for the installed systems during and after functional testing until substantial completion or final acceptance as determined by the VA.
The Commissioning Agent, in collaboration with the control contractor shall include long term trends (greater than 7 days) of integrated system performance once all systems have been commissioned. The Commissioning Agent shall provide analysis of these trends to confirm that the installed systems are stable and reliable

Prepare Systems Manuals
The Commissioning Agent shall work with the design team, contractor and VA to develop Systems Manuals. Manuals will contain system design, operations and sequence information that describes the design intent, operational features and appropriate operational practices necessary to sustain the system operation in accordance with the Design Narrative and the VA’s overall objectives for the facility.
Systems Manual will include sections for each major system included in the commissioning process.

Training Plan Review
The Commissioning Agent shall collaborate with the Prime Contractor and installing subcontractors to develop effective contractor and manufacturer training plans and agendas for general conformance with specifications and VA’s requirements. Observe training for quality of training and for general conformance with the training plan and agenda.
The CxA shall observe contractor and vendor training for critical systems. The Contractor shall provide and the CxA shall maintain all attendance records and associated agendas and presentations for all training sessions.

The Commissioning Agent shall witness and document Systems Functional Performance Testing that was deferred to allow systems to be tested in appropriate seasonal conditions.
Testing will be conducted in accordance with Systems Functional Performance Test Procedures. Testing support will be required from the installing contractors, EMCS Vendor and VA. Requirement for seasonal testing will be incorporated into the project specifications.

1.3 STATEMENT OF BID ITEM(S)
A. Contractor shall completely prepare project site for operations, including demolition and removal of existing site elements, and furnish labor and materials and perform work for the Togus VA Medical Center Bldg 200 and 200E elevator replacement as required by the drawings and specifications. Period of performance is projected to be ____________ calendar days.
B. Work includes general construction, alterations, and certain other items. Contractor shall provide services but not limited to furnish all supervision, labor, equipment, materials, and transportation necessary to accomplish work for the demolition and replacement as required by specifications and drawings.

C. Contractor is responsible for Asbestos Containing Materials (ACM) and Lead Based Paint (LBP) removal and mitigation, including Floor tile, Mastic and Lead Paint removal, where it occurs inside the project area of work. Please see Specifications in Division 2 for estimated quantities and locations. There will be no cost to the Government for any removal, testing, oversight or clearance of ACM.
D. Civil and Sitework: Work includes all labor, material, equipment, and supervision to perform new civil and site work by the construction drawings and specifications. Civil and Sitework includes all site preparation as well as providing new and relocating existing utilities and equipment, providing new site construction, landscaping, and physical security elements.
1. Protection for existing plantings, trees and hardscape to remain shall be installed, maintained and enforced by contractor.
E. Electrical, Telecommunications, and Security Work: Work includes all labor, material, equipment and supervision to perform new electrical, communications and security systems to the spaces renovated and the new additions detailed by the construction drawings and specifications.
Provide access control for all the required doors, coordinate with the door hardware schedule for additional requirements. Provide new cameras, associated devices, and equipment.
F. Mechanical, Plumbing and Fire Protection Work: Work includes all labor, material, equipment, and supervision to perform the required Mechanical, Plumbing and Fire Protection construction work on this project including provide materials and construction equipment as detailed by the construction drawings and specifications.
G. Contractor shall provide new piping, fixtures, valves, fittings, and devices as required by the construction drawings and specifications.
H. Inspections: Comprehensive Third-Party inspections, testing and written reports for all Divisions are required and paid by the General Contractor. Contractor shall employ third party inspection consultants to perform all inspections required by this contract.
I. All new and existing systems within the Scope of Work shall be commissioned (or recommissioned) per Specification 01 91 00 – GENERAL COMMISSIONING
REQUIREMENTS.
J. Contractor shall utilize a Commissioning Agent. The Commissioning Agent is the designated person, company, or entity that plans, schedules, and coordinates the commissioning team to implement the commissioning process

N. The expected construction period duration of days includes all pre-construction activity, construction, and post-construction testing, third-party inspections, Owner inspections, and commissioning. Contractor shall submit a comprehensive schedule of work in compliance with

1. Contractor shall submit and obtain approval for all project submittals including coordination drawings prior to beginning physical construction.
2. Contractor shall perform Commissioning of all new systems and existing systems impacted by the project according to requirements in section 01 91 00 – GENERAL
COMMISSIONING REQUIREMENTS.
3. A detailed commissioning plan is required to ensure that all systems are calibrated, programmed, and all graphic displays are accurate and approved by applicable VA staff.

1.4 SPECIFICATIONS AND DRAWINGS FOR CONTRACTOR
A. Drawings and contract documents may be obtained from the website where the solicitation is posted. Additional copies will be at Contractor's expense.
B. Refer to specifications for installation requirements for each Division. Refer to drawings for estimated quantities and schematic layouts/locations. Contractor shall provide a complete, commissioned, functioning systems. Contractor shall provide all testing reports prior to the start of commissioning. This project will require night and weekend work.
1.5 CONSTRUCTION SECURITY REQUIREMENTS
A. Security Plan:
1. The security plan defines both physical and administrative security procedures that will remain effective for the entire duration of the project.
2. The General Contractor is responsible for assuring that all sub-contractors working on the project and their employees also comply with these regulations.
B. Security Procedures:
1. General Contractor’s employees shall not enter the project site without appropriate badge. They may also be subject to inspection of their personal effects when entering or leaving the project site.
a. All employees of general contractor and subcontractors shall comply with VA security management program and obtain permission of the VA police, be identified by project and employer, and restricted from unauthorized access.
b. Contractors are required to have a badge request form filled out for all personnel (employees, subcontractors, consultants, inspectors, etc.) who will be on-station.
Contractor will bring completed request forms to the project COR for signature.
Contractor will then take signed badge requests to the VAME Police Department for processing at least one (1) week prior to arrival of personnel on station.
Contractor shall arrange with the Police Department for a time to return to pick up temporary badges. Any employee who will perform work on station for longer than six months is required to be issued a flash badge. For any employee requiring a flash badge, the contractor will need to bring the badge request form (with Police Department Approval) back to the COR. The COR will arrange to have employee sponsored in the system. Employee will be notified via e-mail once they are in the system. Employee will then need to arrange a time to complete finger printing and visit the VAME PIV office to be issued their Flash badge. No personnel are allowed to perform any work on station before being issued a badge. No personnel are allowed to perform work on station if their badge is expired.
2. Before starting work the General Contractor shall give two week’s notice to the Contracting Officer so that arrangements can be provided for the employees. This notice is separate from any notices required for utility shutdown described later in this section.
3. No photography of VA premises is allowed without written permission of the Contracting Officer. Patients and staff are not to be photographed at any time.
4. VA reserves the right to close down or shut down the project site and order General Contractor’s employees off the premises in the event of a national emergency. The General Contractor may return to the site only with the written approval of the Contracting Officer.
C. Key Control:
1. The General Contractor shall provide duplicate keys and lock combinations to the
Contracting officers representative (COR) for the purpose of security inspections of every area of project including tool boxes and parked machines and take any emergency action.
2. The General Contractor shall install all permanent cores at completion of the work.
3. Contractor shall provide and install, at all construction entrances, a fully mechanical pushbutton door lock and hardware set (basis of design is Simplex Kaba 5000 Series

Cylindrical Mechanical Pushbutton Lock). The lockset must be fully mechanical with no batteries, power, or electronics required for operation. The lockset must be capable of accepting a cylindrical Sargent 10 Line lock core. Va will provide keyed lock cores for installation.
4. Contractor will provide their own keyed or coded padlock at all construction gates. The VA will also provide and install a padlock to be daisy-chained with contractor lock. The contractor staff will not be issued keys. Locks will be entwined in a fashion that will allow both parties independent access to the site.
D. Document Control:
1. Before starting any work, the General Contractor/Sub Contractors shall submit an electronic security memorandum describing the approach to following goals and maintaining confidentiality of “sensitive information”.
2. The General Contractor is responsible for safekeeping of all drawings, project manual and other project information. This information shall be shared only with those with a specific need to accomplish the project.
3. Certain documents, sketches, videos or photographs and drawings may be marked “Law Enforcement Sensitive” or “Sensitive Unclassified”. Secure such information in separate containers and limit the access to only those who will need it for the project. Return the information to the Contracting Officer upon request.
4. These security documents shall not be removed or transmitted from the project site without the written approval of Contracting Officer.
5. All paper waste or electronic media such as CD’s and diskettes shall be shredded and destroyed in a manner acceptable to the VA.
6. Notify Contracting Officer and Site Security Officer immediately when there is a loss or compromise of “sensitive information”.
7. All electronic information shall be stored in specified location following VA standards and procedures using an Engineering Document Management Software (EDMS).
a. Security, access and maintenance of all project drawings, both scanned and electronic shall be performed and tracked through the EDMS system.
b. “Sensitive information” including drawings and other documents may be attached to e-mail provided all VA encryption procedures are followed.
E. Motor Vehicle Restrictions
1. Vehicle authorization request shall be required for any vehicle entering the site and such request shall be submitted 24 hours before the date and time of access. Access shall be restricted to picking up and dropping off materials and supplies.
2. Parking for General Contractor and its employees shall be in designated areas only.
Contractor to coordinate with Contracting Officer’s Representative.
a. Parking shall be first-come first-serve.
1.6 OPERATIONS AND STORAGE AREAS (FAR 52.236-10)
A. The Contractor shall confine all operations (including storage of materials) on Government premises to areas authorized or approved by the Contracting Officer. The Contractor shall hold and save the Government, its officers and agents, free and harmless from liability of any nature occasioned by the Contractor's performance.
B. Temporary buildings (e.g., storage sheds, shops, offices) and utilities may be erected by the Contractor only with the approval of the Contracting Officer and shall be built with labor and materials furnished by the Contractor without expense to the Government. The temporary buildings and utilities shall remain the property of the Contractor and shall be removed by the Contractor at its expense upon completion of the work. With the written consent of the Contracting Officer, the buildings and utilities may be abandoned and need not be removed.
C. The Contractor shall, under regulations prescribed by the Contracting Officer, use only established roadways, or use temporary roadways constructed by the Contractor when and as authorized by the Contracting Officer. When materials are transported in prosecuting the work, 01 00 00 - 19 vehicles shall not be loaded beyond the loading capacity recommended by the manufacturer of the vehicle or prescribed by any Federal, State, or local law or regulation. When it is necessary to cross curbs or sidewalks, the Contractor shall protect them from damage. The Contractor shall repair or pay for the repair of any damaged curbs, sidewalks, or roads.
D. Working space and space available for storing materials shall be as determined by the COR.
E. Workers are subject to rules of Medical Center applicable to their conduct.
F. Execute work in such a manner as to interfere as little as possible with work being done by others. Keep roads clear of construction materials, debris, standing construction equipment and vehicles at all times.
G. Execute work so as to interfere as little as possible with normal functioning of Medical Center as a whole, including operations of utility services, fire protection systems and any existing equipment, and with work being done by others. Use of equipment and tools that transmit vibrations and noises through the building structure, are not permitted in buildings that are occupied, during construction, jointly by patients or medical personnel, and Contractor's personnel, except as permitted by COR where required by limited working space.
1. Do not store materials and equipment in other than assigned areas.
2. Where access by Medical Center personnel to vacated portions of buildings is not required, storage of Contractor's materials and equipment will be permitted subject to fire and safety requirements.
3. Work on areas outside the building and/or construction site during any given phase shall require off hours work, nights and weekends, as the Medical Center are operated 24/7 for continued operations to support the Medical Center. Work affecting areas outside the designated building and/or construction site (including bringing materials onsite) shall be scheduled with COR per 1.6 and shall be performed after normal work shifts including nights and weekends.
a. The work will have to be carefully planned and phased for minimal disruption to
Medical Center activities. Under no circumstances will any area of the building be shut down and all services must remain operational. Under no circumstances shall there be utility outages, including brief cutovers, anywhere on the campus without the express written consent of the COR. The contractor shall plan and forecast upcoming work which may disrupt normal operations or interfere with staff or patients and report these in the weekly meetings.
b. It may be necessary to postpone or reschedule work to accommodate the needs of the Medical Center. All such time shall be included in the base bid and shall not be billable to the government.
H. Utilities Services: Where necessary to cut existing pipes, electrical wires, conduits, cables, etc., of utility services, or of fire protection systems or communications systems (except telephone), they shall be cut and capped at suitable places where shown; or, in absence of such indication, where directed by COR. All such actions shall be coordinated with the COR or Utility Company involved.
I. Phasing:
1. The Medical Center must maintain its operation 24 hours a day 7 days a week.
Therefore, any interruption in service must be scheduled and coordinated with the COR to ensure that no lapses in operation occur.
