STATEMENT OF WORK

National Institute of Health

National Institute on Minority Health and Health Disparities Creating a Mock-up Web-based Interface to Deliver Mathematical Model-based Estimates for Physical Activity Prescriptions among Breast Cancer Survivors January 01, 2026

1.0 INTRODUCTION

1.1 Background Information

Healthcare providers could play a critical role in improving exercise participation among breast cancer survivors. Studies show that patients prefer receiving information about exercise from their providers, and patient–provider discussions could help increase exercise in patients following treatment. While most clinicians agree that it is important to discuss exercise with their patients, they also report lack of time, low confidence, and safety concerns as limitations to these discussions. Currently, clinicians have no tools to help overcome these limitations and support exercise discussions in clinical settings.

Our lab has developed a mathematical model to estimate individualized breast cancer outcomes (e.g., breast cancer death, life-years gained) associated with different levels of physical activity to help clinicians discuss exercise and write physical activity or exercise prescriptions to breast cancer survivors seen in clinical settings. The results from the mathematical model need to be delivered via a user-friendly interface to help clinicians discuss, prescribe, and make referrals for exercise based on the characteristics, needs, preferences, and values of breast cancer survivors.

The work requirements specified herein include developing a mock-up web interface to support the delivery of mathematical model-based estimates to generate individualized exercise recommendations for breast cancer survivors in the US. These new features will include survivor exercise preferences and readiness, tailored messages, an individualized exercise discussion script, draft clinical notes, draft exercise referrals, letters of medical necessity (LMN), and an individualized exercise patient handout.

The development of a mock-up interface to deliver mathematical model-based estimates to clinicians and breast cancer survivors will require specialized expertise in public health, health communication, intervention design, eHealth user experience and web-based user interface design.

The overall purpose of this SOW is to support clinical research that will lead to favorable health outcomes for all breast cancer survivors in the US. This will be Firm- Fixed-Price purchase order.

1.2 Purpose/Objectives

The purpose of this SOW is to provide specialized non-Research & Development (non-R&D) services to support the NIH/NIMHD Decision Sciences Research Laboratory in developing a mock-up interface to deliver mathematical model-based estimates to support individualized exercise discussions, prescriptions, and referrals for breast cancer survivors in the US.

All work shall be performed in accordance with the established procedures. This requirement supports the NIMHD Intramural Research Program (IRP).

1.3 Scope

Independently and not as an agent of the Government, the Contractor shall furnish all necessary services, qualified personnel, material, equipment, and facilities, not otherwise provided by the Government to successfully deliver the specialized services specified below.

2.0 REQUIREMENTS

2.1 Tasks

The Contractor shall successfully perform the following specialized support Tasks:

• Task 1.0 - Create a mock-up interface to deliver mathematical model-based estimates to clinicians and breast cancer survivors based on NIMHD researcher feedback.

• Task 2.0 – Integrate medical clearance triage items into the mock-up interface outlined in the Exercise in Cancer Evaluation and Decision Support (EXCEEDS).

The items from EXCEEDS will be provided by the NIMHD researchers.

• Task 3.0 - With input from the NIMHD researchers, create a question prompt list to elucidate patient preferences and readiness and generate individually tailored messages for physical activity prescriptions.

• Task 4.0 – Integrate the inputs and outputs provided by the mathematical model developed by the NIMHD researchers into the individually tailored messages developed in Task 3.0.

• Task 5.0 – Develop a list of additional resources (e.g., home exercise instructions) available to support exercise with input from NIMHD researchers.

• Task 6.0 – Include an individualized discussion script to the mock-up web interface based on the model inputs, outputs, question prompt list, and resources available to support exercise.

• Task 7.0 – Draft clinical exercise-related notes for the mock-up web interface.

• Task 8.0 – Include draft exercise referrals and letters of medical necessity (LMN) to the mock-up interface.

• Task 9.0 – Include a section to print out an individualized patient handout based on the inputs, outputs, individualized messages, and resources.

• Task 10.0 - Provide escalated support to address any issues related to the interface.

• Task 11.0 - Deliver the mock-up interface continuously through the period of performance.

• Task 12.0 - Assist the NIMHD research team in planning for publication.
