Statement of Work

26 August 2021

Maritime Vessels Repair

Squadron, Hurlburt Field Florida

1. Scope of Work: The requires repair and maintenance of eleven (11) surface maritime vessels and associated trailers, as listed in

Appendix 1. Work includes preventative maintenance (PM) and repair of non-functioning equipment.

Maintenance and repairs required under this contract Blanket Purchase Agreement

(BPA) shall be performed off-site at the contractor’s location unless the repair is minor and can be performed on-site without interference in base operations. The Contractor’s repair facility must be within 20 miles of the Hurlburt Field front gate. Contractor must have ability to water test engines and vessels as required. Contractor personnel performing the repairs must be certified to work on the part/item/component/vessel being repaired.

The contractor shall not accept any instructions issued by any person employed by the

US Government other than the Contracting Officer Representative (COR), Technical

Point of Contact (TPOC), Contracting Officer (CO) or the Program Manager all acting within the limits of their authority. The contractor shall only perform services IAW this

BPA when directed by an authorized Government official listed on the authorized caller list signed by a contracting officer.

2. Description of Work: The contractor shall provide, as needed; all materials, hardware, replacement parts, equipment, tools and labor necessary to make repairs in accordance with manufacturer’s guidance (hourly/yearly), recommendations and requirements and must follow all federal, state and local guidelines for the disposal of waste. The contractor shall inspect, diagnose, annotate, and after Government approval, make repairs. Equipment to be diagnosed and repaired may include, but is not limited to the following:

a. 2 x 35’ aluminum center console safe boats with T top and foam filled collar;

electrical system (3x batteries for engines and 1 house battery, navigation lights, flood lights, spot lights, general power)

b. 6 x 300hp Mercury Verado engines with Vessel View (remote console)

c. 2 x Ray marine ® navigation system with radar and depth finder with transducer

d. 2 x 300 gallon fuel cells with delivery system (fuel filters)

e. 1 x inflatable Wing 570 boat with center console steering

f. 1 x 60hp Yamaha engine with 6 gallon fuel cell and battery

g. 8 x fiberglass 2016 Sea-Doo SAR (Search And Rescue) with ROTAX ® engines

h. 2 x 35’ Safe Boat trailers (axles, lights)

i. 2 x tandem (in line) Sea Doo ARC trailers (axles, lights)

j. 4 x single Sea Doo ARC trailers (axles, lights)

3. Diagnosis: The contractor shall perform troubleshooting techniques to produce an accurate quote and notify the Authorized Caller of any repair recommendations. Quotes shall be written and provided to the authorized caller prior to commencement of work.

Quotes shall be made by certified mechanics on all aspects of work performed on equipment and the approved price list in the BPA prior to any call being accepted. At a minimum, quotes shall contain the following information:

Call number (provide by authorized caller), date of order, Authorized Caller name

Total amount of hours to be invoiced for troubleshooting or tear down and quote

Estimated number of hours from Approved Price List to complete the repair

Listing of any individual parts or components used for repairs

Parts over $10,000.00, at least three quotes shall be provided

Breakout any other fees not covered under labor or materials

Date when repairs are estimated to be complete

Once a quote is approved by the Government, the Authorized Caller will notify the contractor to commence repairs.

4. Reports: The contractor shall submit a complete legibly written or typed report of the repair to the COR/TPOC within three (3) business days after the maintenance or repair service is accomplished.

5. Voluntary Protection Program: The Air Force is part of the AOSHA VPP. All contractors are required to familiarize themselves with the requirements of the VPP. Information on VPP can be accessed at http://www.osha.gov/desp/vpp/index.html.

6. Base/Installation Specifics: All commercial trucks must enter through the Commercial Gate on Cody Ave off of Independence Road, Fort Walton Beach, FL for inspection.

Prior to going through the Commercial Gate, contractor will have to obtain a visitors pass from the Visitors Center located off of HWY 98. In accordance with the Installation 1st SOSFS Visitor Control Center; base entry procedures must adhere to delivery vehicle process requirements mandated by the 1st SOSFS. Vehicles are subject to inspection. To obtain a base pass from the Visitors Center, the driver will require:

A current Vehicle Registration

A current Proof of Insurance

A valid and current Driver's License for the driver

A valid and current Driver's License or other State Identification for any passenger(s) in the vehicle.

A Base Pass Letter from the Contracting Officer with the contract number, and individuals requiring base access. The base pass request must be submitted to the Contracting Officer at least 10 days prior to scheduled delivery and include the names and driver’s license number of each individual requiring base access. The request shall also include the anticipated installation timeframe for this project.

6.1. Gate Procedures:

Once at Cody Ave. Gate, be prepared to exit the vehicle and allow for vehicle inspection. At this time you may ask for directions to your final destination.

If the final destination requires the driver to leave Cody Ave. Gate, and re-enter the base through an alternate gate, the pass received will be sufficient documentation for entry into both gates, within a pre-established time directed by the Security Forces representative.

Arrival at the appropriate gate after the designated allotted time will result in non-admittance to the installation.

This procedure will occur each time a commercial truck requests access to the base.

6.2. Current Bill of Lading (BOL) to get delivery driver and the vehicle on base. BOL must include the AF installation name, date of anticipated delivery, and specific destination (including building number, street address, and point of contact information).

6.3. Personnel should have the following items when entering an installation:

Current Registration for all vehicles Current Proof of Insurance for all vehicles Valid driver’s license for each driver Valid driver’s license or other state identification for each passenger Depending on the current Force Protection Condition, a government representative may be required to sponsor those people in person at Pass & ID/Visitors Center.

6.4. Prohibitions: Firearms may not be brought on base under any circumstances.

Individuals with criminal records are not allowed on base. Contractor personnel are prohibited from sponsoring guests onto the installation. Contractors must follow all driving regulations while driving on the base. All vehicles are subject to search by the base Security Forces.

6.5. Hazardous Waste: material usage shall be contained and disposed of in a proper manner. For any large spills that may occur, the 1SOCES Environmental Element will be contacted. The contractor must provide legible Safety Data Sheets (SDS) on the work truck and provide to the 1 SOCES Environmental Element, and the government representative (caller) if any large spills occur. If at any time during the contact, the contractor elects to use different lubricant(s), the SDS must be carried on the work vehicle. All hazardous materials shall be used in accordance with the manufacturers’ directions and intended purposes.

6.6. Environmental Requirements: Disposal of excess/outdated hazardous materials or hazardous waste generated shall be removed at job completion or it shall be coordinated through the 1 SOCES Environmental Element. The contractor shall report all petroleum and hazardous spills to the base fire department (911). No fuel (gas or diesel), oil, or lubricant storage tanks will be permitted on Hurlburt Field unless approved and coordinated through 1 SOCES Environmental Element.

6.7. Contract Employees: while on the installation contractor employees shall present a neat appearance and be easily recognized as contractor employees. The contractor shall provide each employee with an identification badge that clearly displays both the employee’s name and the contractor’s name. Identification shall be available prior to employment and shall be worn or attached to the outer garment at all times.

Appendix 1 – List of Vessels & Trailers

a. 2 x 35’ aluminum hull with foam filled collars, T top, center console (2017) Safe

Boats with 3 x 300hp Mercury Verado engines each and 6 wheel aluminum trailers.

b. 1 x inflatable wing boat with a 60hp Yamaha engine and center steering console with trailer.

c. 8 x fiberglass (2016) Sea-Doo SAR (Search and Rescue) with ROTAX ® engines with 2x tandem in line trailers and 4 single trailers.
