Sierra Vista Trail Access Road – Hwy 404 Pipeline Depth Determination – Hydro Vac Excavating

1.0 PROJECT INTRODUCTION

Project Title: Sierra Vista Trail Access Road – Hwy 404 Pipeline Potholing Depth Determination

– Hydro Vac Excavating.

1.1 Scope

In accordance with the terms and conditions of the Contract, the Contractor shall perform the work of this request for the Bureau of Land Management (BLM) as described below.

All work will be completed in accordance with commercial standards and safety practices of hydro vac excavation of major gas pipelines. A New Mexico One Call ticket will be submitted to schedule. Along with coordination of Damage Prevention and Line Rider for the area.

This contract is to provide depth of pipeline or determine greater than 4 feet, whichever comes first, to determine stress calculation to design the amount of cover needed for public access. The location for the access roads will have to cross major utilities; Kinder Morgan four (4) EPNG gas pipelines and three (3) SFPP products pipelines; Magellan two (2) production pipelines. The contractor will be responsible for potholing nine total lines.

EPNG Damage Prevention and SFPP Line Rider for this area, they will need to be on-site for the pothole investigation. A New Mexico One Call ticket will be submitted to schedule The contractor remains fully responsible for the coordination and safety of the work. Problems or confusion shall be brought to the attention of the Contracting Officer (CO) requesting direction.

1.2 Location

This project is in Dona Ana County along Hwy 404 approximately 4-1/4 mile east of Interstate

25. Approximately 32° 0’41.10”N , 106°31’8.00W

1.3 Background

The current location of the Sierra Vista Trail Driveways off Hwy 404 has been requested for relocation by the Bureau of Land Management during the review of the Right of Way (ROW) for

NMDOT Project CN E100203-NM 404 Corridor Project. The relocation is a benefit to the public allowing safer access from NM 404 to the Sothern end of the Organ Mountain Desert

Peaks National Monument and the North end of the Franklin State Park. To continue public access the access roads, need to be relocated and constructed to connect the new driveways to the parking lots and public land access.

1.5 Schedule

Contractor shall not begin until the Contracting Officer (CO) has issued the Start Work Notice.

2.0 PROJECT REQUIREMENTS

2.1 General Requirements

SCOPE OF WORK AND PROJECT SUMMARY/REQUIREMENTS

The project will consist of coordinating with the gas companies to have representative on site during the hydro vacuum work to determine the depth of cover over nine (9) pipelines is a minimum of four (4 feet).

The project will include, but not be limited to, the following:

• Provide timeline of work.

• Secure work area and provide for continued access to the facility.

• Determine depth or that pipe is greater than 4 feet.

Project Schedule

The Contractor shall furnish the project schedule to the Contracting Officer. The purpose of the project schedule is to ensure adequate planning, coordination, scheduling, with BLM and all

ROW holders.

Working Hour Restrictions

All work shall be limited to the weekday hours of 7:00 am to 5:00 pm unless otherwise approved by the Contracting Officer. No work shall occur on Bureau of Land Management holidays or weekends without prior approval.

Preservation of Adjacent Features

Confine all operations to work limits of the project. Prevent damage to natural surroundings.

Restore damaged areas outside the work limits, at no additional expense to the Bureau of Land

Management.

Existing Utilities

The contractor shall coordinate with all utility companies. Beyond the requires pipelines required in this contract.

Accident Prevention

Prior to the Preconstruction Meeting, submit an accident prevention program. The program must be accepted by the CO before any on site work can begin. The program shall comply with OSHA and project requirements. Include the following:

Name of responsible supervisor to carry out the program; first aid procedures; hazards associated with work and methods proposed to ensure property protection, and safety of the public, Bureau of Land Management staff and employees.

Accident Reporting: Reportable accidents, defined as death, occupational disease, traumatic injury to contractor’s personnel, BLM employees or the public; property damage or any accident in excess of $100; and fires must be reported within seven days. Complete an Accident/Property

Damage Report (attached Form CM-22) and forward to the Contracting Officer.

Quality Assurance: Ensure that all employees are physically qualified to perform their assigned duties in a safe manner. Do not allow employees to work if their abilities are impaired. Operators of all equipment shall be able to understand signs, signals, and operating instructions, and shall be capable of operating such equipment.

Accident Prevention Products: Provide the following:

• First aid facilities.

• Personnel protective equipment: Meet requirements of NIOSH and MSHA.

• Emergency instructions, including telephone numbers and reporting instruction for ambulance, physician, hospital, fire department and park police. Place in conspicuous locations at the worksite.

Training: Provide training for first aid and hazardous material handling and storage.
