About this file
Summary: Statement of Work - Forest-Wide Well and Septic Tank Decommissioning
This Statement of Work (SOW) for the U.S. Forest Service, Southwestern Region 03 Prescott National Forest, establishes requirements for abandoning wells and closing septic tanks across the forest. The contractor must furnish all labor, materials, equipment, and supervision to complete well abandonment per the Arizona Department of Water Resources (ADWR) Well Abandonment Handbook (2008 Edition) and septic tank closure per Arizona Administrative Code R18-9-A309D. The performance period is 120 calendar days from Notice to Proceed. Base bid work includes abandoning eight ADWR-registered wells (six dug wells and two drilled wells) and two non-ADWR well shafts across four geographic zones (Horsethief Basin, South Prescott, West Prescott). Optional work includes abandoning five additional wells and closing two septic tanks. The contractor must submit a construction schedule with detailed task breakdown and key milestones, as well as proof of well driller qualifications meeting ADWR/ADEQ requirements. All ADWR-qualified wells require submission of DWR Form 55-58 Well Abandonment Completion Reports within 14 days of completion. The Government furnishes road grading for Wilhoit Well Shaft location and ADWR Form 55-38 with associated fees.
Well abandonment specifications require removal of casing and/or re-perforation of existing casing followed by filling with cement grout, bentonite grout, or sand-cement materials per ADWR-approved methods. For dug wells requiring collapse, the work calls for filling approximately halfway with earthen material, pouring a 2-foot cement/grout mixture, then filling remaining void with earthen material to surface. Septic tank closure requires removing sewage, disconnecting mechanical components, removing or collapsing tank tops, punching holes below seasonal groundwater tables, filling with earth or approved material, and regrading surfaces for drainage. The SOW specifies protection of natural resources, prevention of ground disturbance outside work limits, site cleanup requirements, and protocols for discovering archeological sites or human remains. All well and septic locations are provided with GPS coordinates, field photographs, access details, and existing condition documentation throughout the attachments.

1.10 ARCHEOLOGICAL SITES AND DISCOVERY
If previously undocumented cultural resources are encountered during ground-disturbing activities, all work must immediately cease within one hundred (100) feet until a USFS archaeologist has documented the discovery and evaluated its eligibility for the National Register of Historic Places (NRHP) in consultation with the Arizona State Historic Preservation Office (SHPO). Work must not resume in the area without authorization from the USFS. Secure the area and relocate work to another area of the project.
If the unanticipated discovery involves human remains and/or associated objects on USFS lands, all work must immediately cease within one hundred (100) feet of the discovery and the USFS must be notified immediately of the discovery. The area of the find must be clearly
7 | P a g e delineated, protected, and secured until USFS staff have been contacted and have had an opportunity to evaluate the nature and significance of the discovery. All discoveries on USFS lands will be treated in accordance with the Native American Graves Protection and Repatriation Act (PL 101-601; 25 United States Code 3001–3013), and work must not resume in this area without authorization from the USFS.
1.11 PROTECTION OF NATURAL RESOURCES

2.0 PROJECT SCOPE
2.1 SCOPE OF WORK
The Contractor shall furnish all supervision, materials, equipment and vehicles, tools, parts, labor, transportation, lodging, and miscellaneous items to abandon wells across the PNF, and close septic tanks, in accordance with the drawings, specifications, and other attachments referenced and contained herein.
No changes to the specifications, terms, or conditions of this statement of work shall be made without the prior approval of the Contracting Officer, in writing.
2.1.1 Dug Wells
For dug wells that require collapsing, fill in approximately halfway with earthen material, then pour 2-foot cement/grout mixture. Bagged cement mix is acceptable. Then fill remaining void with earthen material to surface. Filling in the hole with the broken apart concrete is acceptable only on the second layer (above the cement/grout plug).

C. EXECUTION
a. GENERAL
i. Place materials or mixtures with sufficient pressure to fill all voids, including all annular spaces, and displace water from the well.
ii. When grout is placed, used a tremie pipe and raise at a rate that will always keep the bottom of the pipe no more than 3 feet above the grout level.
iii. When high-solids bentonite chips and pellets are used, hydrate to the manufacturer’s specifications.
iv. Provide materials and mixtures exceeding approximately 30% of the casing volume.
b. SURFACE SEAL REQUIREMENTS
i. Surface Casing Removal Option, ADWR Section III-A, Option 1
1. If the casing is removed from the top 20 feet of the well, install a cement grout plug extending from two feet below the land surface to a minimum of twenty feet below the land surface.
2. Backfill well above the top of the cement grout plug to the original land surface.
ii. Surface Casing Non-Removal Option, ADWR Section III-A, Option 2
1. If the casing is not removed from the top 20 feet of the well, install a cement grout plug extending from the top of the casing to a minimum twenty feet below the land surface.
2. Fill the annular space outside the casing with cement from the land surface to a minimum twenty feet below the land surface.
iii. Backfill well above the top of the cement grout plug to the original land surface.
c. STANDARD ABANDONMENT METHODS
i. ADWR Section IV-A, Standard Method 1
1. Remove the entire length of well casing.
ii. ADWR Section IV-A, Standard Method 2
1. Re-perforate the entire length of the casing, a minimum of two cuts per foot, from 20 feet above the highest historic water level to the total depth of the well.
iii. Fill the well completely with neat cement, cement-bentonite grout, or, except where free-product contamination is present, high-solids bentonite grout (granular or powder mixtures) with a minimum of 25% solids by weight.
iv. Materials or mixtures must be emplaced under sufficient pressure to fill all voids, including all annular space(s), and displace water from the well.
v. A tremie pipe must be used to emplace the ground from the bottom up.
The end of the tremie pipe must remain near the rising grout surface, as the grout is pumped into the well.
d. ALTERNATIVE ABANDONMENT METHODS
16 | P a g e
i. Use ADWR-approved alternative abandonment methods when it is determined that the disturbance of the casing and/or gravel packed zones would negatively influence the sealing of the well.
ii. ADWR Alternative #1: Wells that do not penetrate aquifers, including wells that have gone dry, and no vadose zone contamination issues exist.
1. Fill well with one or more of the following mixtures:
a. Clean fine sand.
b. Cement grout (including neat cement grout, cement-bentonite grout, and sand-cement grout).
