Statement of Work
Bureau of Land Management Arizona State Office
1. OBJECTIVE
The Bureau of Land Management (BLM), Arizona State Office (AZSO), located at One North Central Ave, Phoenix AZ, 85004 is looking to secure services to repair the Physical Access Control System (PACS) software applications to ensure it is running properly.
2. SCOPE
Contractor shall provide services and personnel to include labor, equipment, and materials (unless otherwise provided herein) necessary to meet the requirements of this Statement of Work (SOW). The PACS system has 37 card readers, two client servers operating AMAG Symmetry, 16 HID PAMs and all associated components.
The migration of the current HID program from the Firebird database to the SQL Database may be needed to allow more space (SQL Database is larger). This migration may need to take place to ensure all programs are functioning, so all new commands and reader programming are pushed and uploaded to the end points.
The government is seeking a contractor to perform diagnostics and repair of the AMAG Symmetry client as well as the HID program that handles the validation of PIV certification, and any other programs needed to ensure the proper function of the PACS usage. The contractor will provide a one-year warrantee for all work completed.
Contractor will also provide any licenses needed for all components to operate and meet all product manufacturer’s recommendations, the attachments to the Scope of Work, and applicable industry standards. The contractor shall work closely with the Contracting Officer’s Representatives (CORs) to ensure that the objective of this SOW is executed in accordance with the terms and conditions of this contract. Work shall be completed in a professional manner according to standard industry practices.
All materials and/or parts must meet FIPS-201 and be General Schedule Agency (GSA) approved and approved by the COR prior to procurement and installation.
3. SPECIFICATIONS
The Contractor shall maintain, update, monitor and repair as needed the systems identified herein.
1. Physical Access Control System (PACS)
a. AMAG Symmetry

2. CONTRACT TYPE
The government anticipates awarding one Firm Fixed Priced (FFP) contract to the vendor that meets all the requirements herein.
3. PERIOD OF PERFORMANCE
Services need to be started within 5 days from the award of this contract and should be completed within 14 days from the start of this contract. Unforeseen issues may allow an extension of the completion of the contract.
4. DELIVERY AND PERFORMANCE LOCATION
Work shall be accomplished during normal business hours. Any work required to be performed outside normal business hours shall be discussed and coordinated with the Contracting Officer Representative (COR) prior to work being performed.
All work shall be completed Monday – Friday from 7:00am – 4:00pm MST.
5. PROJECT MANAGEMENT
The contractor will provide overall project management and coordination to remove, replace, and upgrade all identified items.
The Contractor shall identify and provide a single point of contact (POC) to provide overall management and supervision during the period of performance of this contract.
All pertinent communications should be written or confirmed in writing. The single POC shall attend and participate in necessary progress meetings.
6. SITE VISIT
A site visit is not required, but it is strongly encouraged that potential offerors attend the stie visit to prior to submitting a proposal.

7. KICK-OFF MEETING
The Contractor and the COR shall meet for a mandatory kick-off meeting upon execution of the award. During this meeting all questions shall be addressed along with determination of execution of the requirements. The contractor shall provide to the COR a list of personnel who will be working at the job site within three (3) calendar days from award. The contractor shall work with the COR to coordinate access to each location.
8. INSTALLATION SERVICES
The contractor shall coordinate all delivery and repair schedules with the COR. The contractor is responsible to ensure that all phases of the contract are managed and communicated with the COR. It is the contractor's responsibility to provide an adequate work force to ensure that the required schedule is met.
Any missing or damaged products must be replaced within a four-week period. The contractor shall reimburse the BLM for costs associated with damage cause by the installers and any repairs due to the installation effort. The contractor will be responsible for any damage to the building which occurs during delivery and repairs.
Any unexpected conflicts during delivery and repair must be brought to COR for resolution.
Any and all risk of loss for any Contractor-owned and or leased materials, inventory, furnishings, fixtures, equipment, and supplies are the sole responsibility of the Contractor.
9. GOVERNMENT HOLIDAYS
The Contractor agrees to observe the following Government holidays for onsite personnel:
New Year’s Day Labor Day Martin Luther King’s Birthday Columbus Day President’s Day Veteran’s Day Memorial Day Thanksgiving Day Juneteenth Day Christmas Day Independence Day
The Contractor shall observe any other day designated by Federal statute, Executive Order or the Presidential proclamation.
When any such day falls on a Saturday, the preceding Friday is observed; when any such day falls on a Sunday, the following Monday is observed. All personnel assigned to perform under this contract shall limit their observation of holidays to those set forth above.
10. RECYCLING, TRASH REMOVAL AND DISPOSAL
While working onsite, the contractor shall maintain a hazard-free workspace, and clean up and dispose of trash/waste material daily.
11. STANDARDS OF CONDUCT
The contractor shall maintain satisfactory standards of employee competency, conduct, cleanliness, appearance, and integrity and shall be responsible for taking such disciplinary action with respect to employees as many be necessary. The BLM reserves the right to direct the contractor to remove an employee from the worksite for failure to comply with the standards of conduct. The contractor shall immediately replace such an employee to maintain continuity of services at no additional cost to the
BLM.
12. SAFETY, HEALTH AND ENVRONMENTAL MANAGEMENT
The contractor shall take all reasonable and proper safety precautions to prevent death or injury to any person or damage to any property at the specific location, and in particular all equipment used by the Contractor shall be used in such a manner and maintained so as to minimize the danger of accident, death, injury, loss or damage arising from the use of such equipment. In addition to relevant statutory requirements, standards and other provisions of this Contract, the Contractor shall have the following requirements:
The Contractor’s personnel shall be knowledgeable with and adhere to all relevant occupational health and safety regulations.
Ensure that the Contractor’s personnel comply with all safety procedures and requirements.
Ensure that the Contractor’s personnel are adequately trained and instructed in the safe and correct usage, handling and operation of materials and equipment relevant to the task order requirements.
Ensure the Contractor’s personnel are certified as having completed occupational health and safety training and have been issued all the necessary Personal Protection Equipment (PPE).
13. QUALITY CONTROL PLAN
The contractor shall develop and maintain an effective quality control program to ensure requirements are provided in accordance with this SOW. The plan shall address the contractor's processes to ensure quality control during performance of services. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor’s quality control program is the means to assure work complies with the SOW and terms and conditions on contract/task orders. Throughout the life of this contract any proposed changes will be provided to the CO for written approval.
14. INSPECTION
The COR will be responsible for inspection and acceptance upon completion of work.
15. WARRANTY
Disclaimers and Limitations: Manufacturer's disclaimers and limitations on product warranties do not relieve contractor of warranty on work that incorporates products. Manufacturer's disclaimers and limitations on product warranties do not relieve suppliers, manufacturers, and subcontractors required to countersign special warranties with contractor.
Related Damages and Losses: When correcting failed or damaged warranted product, the contractor will be responsible for removing and replacing such products.
Reinstatement of Warranty: When work covered by a warranty has failed and been corrected by replacement or rebuilding, the warranty must be reinstated by written endorsement.
Replacement Cost: Upon determination that work covered by a warranty has failed, work shall commence to replace or rebuild work to acceptable conditions, in compliance with the requirements of the contract terms and conditions. The contractor is responsible for the cost of replacing or rebuilding defective work, regardless of whether BLM has benefitted from use of work through a portion of its anticipated useful service life.
Rejection of Warranties: BLM reserves the right to reject warranties and to limit selection to products with warranties in compliance with the requirements of the contract.
Contract Documents: Where contract documents require special warranty, or similar commitment on completed work, the Government reserves the right to refuse to accept work, until the contractor presents written evidence that all entities required to countersign such commitments have done so or are willing to do so.
16. CONTRACTING OFFICER AUTHORITY
The BLM Arizona State office CO shall be the only individual authorized to modify any terms and conditions of this contract.
17. CONTRACTING OFFICER REPRESENTATIVE (COR)
A Contracting Officer Representative (COR) will be appointed to administer this contract, and inspect and accept all work on behalf of the Government. This designation does not include authority to modify or amend the terms of this contract. In no event shall any understanding or agreement, contract modification, change order, waiver, or other matter in deviation from the terms of this contract between the Contractor and a person other than a warranted CO be effective or binding upon the
Government. All such actions must be formalized by a proper contractual document executed by the CO. The contractor shall not take direction of any contract revisions without the written authority of the CO.
All technical questions concerning this requirement shall be directed to the designated COR. The CO and the COR are the sole point of contacts for all technical and contractual discussions. The contractor shall take no direction verbal or otherwise from any other BLM employee other than the CO and/or
COR.
The Contractor shall be responsible for notifying the COR of any work or situation to be performed in a functional area under his/her control that would tend to disrupt the conduct of normal Government business or the normal operation of equipment or systems. The Contractor shall notify the COR at least two working days in advance of such work. Notification of such work shall include the type of work to be done and the estimated completion date. The Contractor shall reschedule any work that the COR deems necessary to avoid unnecessary disruptions in the Government’s business.
22. KEY PERSONNEL
The contractor shall assign a Project/Program Manager designated as key personnel on this contract.
the contractor shall make no substitution of this key personnel unless the substitution is necessitated by illness, death, termination of employment, or otherwise approved by the CO. The contractor shall notify the CO within one (1) business day after the occurrence of any of these events and provide information required below. The contractor shall provide an explanation of the circumstances necessitating the proposed substitution, complete resume for the proposed substitutes and any additional information requested by the CO. Proposed substitutions should have comparable qualifications to those of the persons being replaced. The CO will notify the contractor within one (1) business day after receipt of all required information of the decision on substitutions.
23. BADGING REQUIREMENTS
All contractors will be issued a visitors’ badge while on location. The government COR will work with the contractor and COR/POC for each government location to ensure that all contract employees have a visitors’ badge, and all performance is being completed in accordance with this contract.

The Contractor shall follow Section 889 of the John S. McCain National Defense Authorization Act (NDAA) for Fiscal Year 2019 (P.L. 115-232 [PDF, 789 pages]) and the Federal Acquisition Regulation (FAR) Case 2018-017 prohibit the purchase of covered telecommunications equipment and services from vendors who sell products containing spyware. These devices could pose a threat to U.S. security by spying on or disrupting communications within the U.S. when procuring materials for this project.
