STATEMENT OF WORK

FOR

FORUM 3 ALERT SYSTEM SECONDARY CRASH NET (SCN)

AT MACDILL AFB, FL

13 JUN 2023

SECTION 1.0 DESCRIPTION OF SERVICES/GENERAL INFORMATION

1.1 SCOPE OF WORK (SOW):

1.1.1. The contractor will engineer, furnish, install, test, and place into service an Emergency Conferencing System to meet SCN requirement IAW AFMAN 13-204v2, paragraph 4.2.2.3.

Vendor/items must be listed on the DoDIN Approved Products List in order to be considered for award.

1.2. SPECIFIC TASKS:

1.2.1. The contractor shall conduct an engineering site survey to determine installation capabilities.

1.2.2. The contractor shall engineer, furnish, install, test, and place into service a SCN Emergency Conference System.

1.2.3. All necessary equipment, tools, materials, supplies, transportation, management, and other incidentals shall be provided.

1.2.4. The Emergency Conference System shall be equipped with 30 ports and sessions to alert and track calls to first responders and support key emergency agencies. The designated number of telephone stations shall be installed and connected to all required agencies IAW AFMAN 13-204v2, paragraph 4.2.2.3.5 and additional agencies laid out by 6th OSS/OSAA. All equipment, supplies, and materials provided shall be new and not refurbished.

1.2.5. The contractor and 6th OSS/OSAA will agree upon a schedule that denotes major activities and includes start and completion dates for this project during the Pre-Installation Survey/Teleconference. The contractor shall identify any changes to the schedule by providing an updated schedule and include justification and requests for approval for all schedule modifications.

1.2.6. The contractor shall remain in compliance with all Federal and MacDill AFB security and safety laws, regulations, policies, and requirements.

1.2.7. At the end of each day, the contractor shall keep the working area in a neat and clean state.

Equipment and materials required to complete the work effort may remain on site as long as they are organized and stored in a manner that does to cause a safety hazard.

1.2.8. The contractor shall perform all work during normal duty hours of the site. Any work outside of normal hours shall be coordinated at least three days in advance with 6th OSS/OSAA.

1.2.9. The contractor shall provide training on the system to site personnel. This shall include operator training as well as maintenance and troubleshooting.

SECTION 2.0. SERVICES SUMMARY:

2.1. All work the contractor is responsible for accomplishing shall be agreed upon in accordance with this SOW and be completed within the specified date agreed upon by both parties.

2.2. Secondary Crash Net System for 6th OSS/OSAA, shipping of materials and fabricated set to destination, installation, and setup and lastly training on the overall operations and troubleshooting of all deliverables at final destination.

SECTION 3.0. GENERAL INFORMATION:

3.1. Place of Delivery

3.1.1. The equipment will be delivered to 7719 Hangar Loop Drive, Hangar 3, MacDill AFB, FL 33621.
