STATEMENT OF WORK

Septic Tank Pumping and Disposal of Sewage Effluent Park Wide

February 6, 2017

1.0 GENERAL REQUIREMENTS:

A. The intention of this solicitation is to obtain Septic Tank and Vault pumping, hauling and disposal services at Valley Forge National Historical Park, King of Prussia, PA.

Tank volumes indicated in Section 2.B are the tank design capacities. They are provided for estimating purposes only.

B. Valley Forge National Historical Park has a total of nine (9) holding tanks that require the routine pumping and disposal of sewage. On average the annual basis 270,000 gallons of sewage is pumped from these locations. The remaining thirteen

(13) septic tanks require only periodic pumping/disposal on a will-call basis removing approximately an additional 10,000 gallons of sewage. Frequency, manhole cover numbers and capacities are specified in Section 2.B. The contractor shall be responsible for providing the personnel and equipment to perform the work, as well as ensuring that all required licensing and permits to pump, transport and properly dispose of sewage is obtained and maintained. Pumping trucks must be capable of metering the sewage removed. Maximum sewage pumped in a year would be 280,000 gallons

C. Applicable Regulations:

The Contractor shall be responsible ensuring compliance with all applicable Federal, State, and local regulations governing the pumping, transportation, treatment and disposal of sewage. Additionally the contractor shall be responsible for following all applicable Environmental, Health and Safety regulations, including the:

Title 25 Consolidated Statute Chapter 95- WASTEWATER TREATMENT

REQUIREMENTS

The contractor shall also be required to properly implement cleanup procedures and make all applicable notifications to park personnel and the Commonwealth of Pennsylvania in the event of a sewage spill. The contractor shall be licensed by the Commonwealth to transport sewage and shall have all transportation vehicles properly marked in accordance with the State Law.

D. This contract consists of a Base Year and 4 Option Years.

2.0 SCOPE:

A. This scope of work has been prepared from the current knowledge of the physical conditions and previously disclosed surveys and work activities. It is the responsibility of the contractor to inspect all existing conditions prior 'to commencement of work and also comply with the Valley Forge National Historic Park's Health and Safety Plan, which is to include all Local, State, and Federal Codes and Regulations during the execution of the work that is to be performed in association with this project

B. Scheduled Pumping

1. Monthly for 12 months

a. The contractor shall provide all personnel, equipment and incidentals to provide monthly pumping of the following holding tank for 12 months between October 1st and September 30th:

b. Approximately 120,000 gals are pumped from this location annually

2. Monthly for 9 months

a. The contractor shall provide all personnel, equipment and incidentals to provide monthly pumping of the following holding tanks for 9 months between March 1st and November 30th.

b. Approximately 130,000 gals are pumped from these two locations for the 9-month period

3. Two services only: October and April

a. The Contractor shall provide all personnel, equipment and incidentals for the bi-annual pumping of the following septic tanks. Once in October and once in April.

b. For the locations below the contractor shall contact the Park 24 hours in advance so that the Park can schedule an NPS employee to accompany the

Cuyahoga Valley National Park – Replacing Roofs - SOW Page 4 contractor to each site. The NPS employee will verify receipt of services by signing the manifest on-site. Failure of the contractor to secure the signature of an NPS employee at the time of service may result in non-payment.

C. Will Call Basis Pumping

a. The Contractor shall provide all personnel, equipment and incidentals for the periodic pumping (we-will-call) of 13 additional septic tanks that could produce up to 10,000 gal/year.

b. For Will Call pumping the Park will contact the contractor 24 hours in advance to schedule the service and will have an NPS employee accompany the contractor to each site. The NPS employee will verify receipt of services by signing the manifest on-site. Failure of the contractor to secure the signature of an NPS employee at the time of service may result in non-payment.

c. Contractor shall provide price/gal for servicing these locations at the time of quote.

D. Hours of Operation

a. All contractor work shall be performed on weekdays between the hours of 7:30 a.m. and 3 p.m.

b. No work shall be performed by the contractor on Federal Holidays.

c. For all work performed for the specified monthly service locations the contractor shall contact the Park 24 hours in advance so that the Park can schedule an employee to accompany the contractor to each site.

d. The NPS employee will verify receipt of services by signing the manifest on-site. Failure of the contractor to secure the signature of an NPS employee at the time of service may result in non-payment.

3.0 UNIT PRICING/BILLING/INVOICING:

A. The contractor shall invoice on a monthly cycle for the total number of gallons pumped and disposed of during that period and shall include the following:

a. The contract number

b. A unique invoice number

c. The invoice date

d. The actual service date(s)

e. The vendor name and address

f. The Park name, location serviced and address

g. Description of services

h. Invoiced amount

B. Manifest(s) will include the:

a. Location of tank by common description

b. Date of service

c. Number of gallons pumped/disposed of from each tank

d. A unique manifest number

e. Signature of an NPS employee at the time of the service

4.0 SAFETY REQUIREMENTS:

A. While performing work, the contractor shall comply with OSHA standards and all standard safety practices and procedures applicable to the work.

B. Visitor safety is imperative - all areas of work must maintain a high safety alert for visitor traffic utilizing signage or orange fencing or cones.

C. All work must comply with applicable State and Federal codes and regulations.

D. OHSA/ANSI safety vest or shirts must be worn at all time while working on park property.

E. Contractor employees are to be dressed appropriately at all times, No cut off shorts or sleeveless t-shirts. Only Plain OHSA, ANSI approved safety shirts or shirts with contractors Name/Log printed on them can be worn

F. While performing work, the contractor shall comply with OSHA standards and all standard safety practices and procedures applicable to the work.

G. Visitor safety is imperative - all areas of work must maintain a high safety alert for visitor traffic utilizing signage or orange fencing or cones.

H. All work must comply with applicable State and Federal codes and regulations.
