STATEMENT OF WORK
FOR
CUSTODIAL SERVICES
BLM, Little Snake Field Office

STATEMENT OF WORK
FOR
CUSTODIAL SERVICES
1. DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, equipment, and labor necessary to ensure that custodial services are performed at
Little Snake Field Office (LSFO) in a manner that will maintain a satisfactory facility condition and present a clean, neat and professional appearance. Note: The Government will provide paper products including but not limited to toilet paper and paper towels, trash bags, and can liners.
1.1. BASIC CLEANING SERVICES. The contractor shall accomplish all cleaning tasks to meet the requirements of this Statement of Work (SOW). The minimum cleaning frequencies are established in Appendix A, Workload Factors.
1.1.1. Maintain Floors. All floors, except carpeted areas, shall be swept, dust mopped, damp mopped, wet mopped, dry buffed, and spray buffed, as needed, to ensure they have a uniform, glossy appearance and freedom from dirt, debris, dust, scuff marks, heel marks, other stains and discoloration, and other foreign matter. Baseboards, corners, and wall/floor edges shall also be clean and free from dirt, debris, dust, scuff marks, heel marks, other stains and discoloration, and other foreign matter. All floor maintenance solutions shall be removed from baseboards, furniture, trash receptacles, etc. Chairs, trash receptacles, and other moveable items shall be moved to maintain floors underneath these items. All moved items shall be returned to their original and proper position.
1.1.2. Remove Trash. All trash containers shall be emptied and returned to their initial location. Boxes, cans, papers, etc. placed near a trash receptacle and marked “TRASH” shall be removed, any recyclable items are to be placed in marked recycling containers.
Any obviously soiled or torn plastic trash receptacle liners shall be replaced. The trash shall be deposited in the nearest outside trash collection container. Trash receptacles shall be left clean, free of foreign matter, and free of odors. Note: BLM employees are responsible for processing and removing all paper and cardboard recycling.
1.1.3. Public Ashtrays and Urns. All public ashtray urns shall be emptied. Ashtrays shall be clean and free of ashes, odor, and stains. Clean all public urns and replace sand.
Urns shall be repainted when necessary to maintain clean appearance.
1.1.4. Interior Glass/Mirrors. All foyer or entry way interior glass, including glass in doors, partitions, walls, display cases, pictures, directory boards, etc. shall be clean.
There shall be no tract of film, dirt, smudges, water, or other foreign matter.
1.1.5. Drinking Fountains. Shall be clean and disinfect all porcelain and polished metal surfaces, including the orifices and drain, as well as exterior surfaces of fountain.
Drinking fountains shall be free of streaks, stains, spots, smudges, scale, and other obvious soil.
1.1.6. Carpets. All carpeted areas shall be vacuumed. After vacuuming, the carpeted area shall be free of all visible dirt, debris, litter and other foreign matter. Any spots shall be removed by carpet manufacturer’s approved methods as soon as noticed. All tears, burns, and raveling shall be brought to the attention of the Contracting Officer
Representative (COR). Area and throw rugs are included to receive this service.
1.1.6.1. Clean Carpets. Spot clean or shampoo dirty carpets over an area of 2 square feet or less. Spots must be removed immediately.
1.1.7. Vacuum and Clean Floor Mats. Vacuum and clean interior and exterior floor mats. After vacuuming or cleaning, mats shall be free of all visible lint, litter, soil and other foreign matter. Soil and moisture underneath mats shall be removed and mats returned to their normal location.
1.1.8. General Spot Cleaning. Perform spot cleaning on a continual basis utilizing the appropriate maintenance or disinfectant solution. Spot cleaning includes, but is not limited to removing, or cleaning smudges, fingerprints, marks, streaks, spills, etc., from washable surfaces of all walls, partitions, vents, grillwork, doors, door guards, door handles, pushbars, kickplates, light switches, temperature controls, vending machines, fridges, microwaves, furniture and fixtures. After spot cleaning, the surface shall have a clean, uniform appearance, free of streaks, spots, and other evidence of soil. Note: BLM employees are responsible for all dish washing and the cleaning of the interior of the fridge’s and recyclable materials.
1.1.9. General Dusting. All horizontal surfaces up to six feet in height must be dusted or cleaned to eliminate dust collection.
1.2. BASIC RESTROOMS/SHOWER & LOCKER ROOMS CLEANING
SERVICES. The contractor shall accomplish all cleaning tasks to meet the requirements of this SOW and the Service Delivery Summary (SDS).
1.2.1. Clean and Disinfect. Completely clean and disinfect all surfaces of sinks, toilet bowls, urinals, lavatories, showers, shower curtains, shower mats, dispensers, plumbing fixtures, saunas, partitions, dispensers, doors, walls, and other such surfaces, using a germicidal detergent. After cleaning, receptacles will be free of deposits, dirt, streaks, and odors. Disinfect all surfaces of partitions, stalls, stall doors, entry doors, (including handles, kickplates, ventilation grates, metal guards, etc.), and wall areas adjacent to wall mounted lavatories, urinals, and toilets.
1.2.2. Descale Showers, Toilet Bowls and Urinals. Descaling shall be performed monthly as a minimum and as often as needed to keep areas free of scale, soap films, and other deposits. After descaling, surfaces shall be free from streaks, stains, scale, scum, urine deposits, and rust stains.
1.2.3. Sweep and Mop Floor. After sweeping and mopping, the entire floor surface, including grout, shall be free from litter, dirt, dust and debris. Grout on wall and floor tiles shall be free of dirt, scum, mildew, residue, etc. Floors shall have a uniform appearance without streaks, swirl marks, detergent residue, or any evidence of soil, stain, film or standing water. Moveable items shall be tilted or moved to sweep and damp mop underneath. Floors shall be stripped, scrubbed, waxed, etc., and as necessary to maintain sanitary conditions and a clean, uniform appearance.
1.2.4. Stock Restroom Supplies. Contractor shall ensure restrooms are stocked sufficiently so that supplies including soap for the soap dispensers do not run out.
Supplies shall be stored in designated areas. No overstocking shall be allowed. If supplies run out prior to the next service date, contractor shall refill within twenty-four hours of notification. If paper are Government supplied products are close to running low notification to the COR will be made immediately to ensure Government supplied products are ordered and stocked before paper products run completely out.
1.3. PERIODIC CLEANING SERVICES.
1.3.1. Strip, Scrub, Seal, and Wax Floors. Strip, scrub, seal, and wax floors as necessary to maintain a uniform glossy appearance. A non-skid wax is required. A uniform glossy appearance is free of scuff marks, heel marks, wax build-up, and other stains and discoloration. To be performed at a minimum of one time every twelve months.
1.3.2. High Dusting. All horizontal surfaces over six feet in height must be dusted or cleaned to eliminate dust collection. Not limited to emergency lighting, door jams, lighting fixtures, etc. To be performed at a minimum of two times every twelve months.
1.4 CLEANING PRODUCTS REQUIREMENTS
1.4.1. Selection of Cleaning Products. The contractor shall make careful selection of cleaning products and equipment to comply with the following:
1. Purchase and use green cleaning products and hand soaps that have the following environmentally responsible features:
a. Non-toxic
b. Contain no phosphates
c. Non-corrosive
d. Contain no ozone depleting compounds
e. Contain no known carcinogens
f. Contain no chlorine bleach
g. Contain no butoxyethanol
h. Contain no quaternary ammonium compounds
i. Contain no petroleum distillates
j. Non-flammable
k. Concentrated for use in cold water
l. Safe volatile organic compound levels when diluted; and
m. Packaged ecologically in recycled content cardboard boxes and plastic containers to the maximum extent practical
2. Eliminate use of harsh chemicals and the release of irritating fumes.
3. All cleaning products must be Green Seal certified, (http://www.greenseal.org/FindGreenSealProductsAndServices.aspx), EPA Safer
Choice, (http://www2.epa.gov/saferchoice) formerly known as EPA Design for the Environment certified, or USDA Biopreferred certified
(http://www.biopreferred.gov/BioPreferred/faces/catalog/Catalog.xhtml#).
1.4.2. Selection of Paper Products and Trash Bags and Can Liners. The Government shall select and purchase consumable products supplied in the owned space that are fully compliant with the recovered/recycled content percentages specified in the
Environmental Protection Agency’s Comprehensive Procurement Guidelines (CPG) or be
Green Seal certified.
1. Those products include, but are not limited to the following:
a. Toilet paper
b. Paper towels
c. Facial tissue
d. Trash bags and can liners

4. Trash bags and can liners that meet the biobased content established by the U.S.
Department of Agriculture’s BioPerferred Program and are a USDA Certified
Biobased Product are also acceptable.

5. The Government shall comply with the provisions in the Federal Acquisition
Regulations clauses 52.223-4 “Recovered Material Certification” and 52.223-1
“Biobased Product Certification”.
2. PERFORMANCE REQUIREMENTS SUMMARY. The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance standards briefly describe the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The Government will provide paper products including but not limited to toilet paper and paper towels, trash bags, and can liners. The Government will provide the contractor storage space for supplies and equipment.
4. GENERAL INFORMATION.
4.1. QUALITY ASSURANCE. The government will periodically evaluate the contractor’s performance in accordance with the Inspection Checklist.
4.2. GOVERNMENT REMEDIES. The CO shall follow FAR 52.212.4, Contract
Terms and Conditions-Commercial Items (May 2015), for contractor’s failure to perform satisfactory services or failure to correct non-conforming services.
4.3. HOURS OF OPERATION. Work will be performed between Monday through
Saturday, 1700-0600. Because of security and monitoring requirements, the contractor will provide a work and cleaning schedule, and a list of employees and supervisory personnel to the CO for approval and distribution. The work schedule must show expected work days and times; either (Monday/Wednesday/Friday or
Tuesday/Thursday/Saturday). In the event this office is placed under threat by the
Homeland Security Office, the contractor must rearrange the work schedule to three (3) days a week (Monday through Friday) between the hours of 6:00 a.m. and 6:00 p.m.
unless other arrangements can be made with the COR. Should it be necessary to deviate from the approved schedule, the Contractor must notify the CO prior to any deviation.
OBSERVANCE OF LEGAL HOLIDAYS:
(a) The Contractor hereby agrees to observe the following Government holidays:

Independence Day Christmas Day
Labor Day Martin Luther King's Birthday
(b) Also, any other day designated by Federal Statute, any other day designated by Executive Order, and any other day designated by the President's proclamation. When any such day falls on a Saturday, the preceding Friday is observed; when any such day falls on a Sunday, the following Monday is observed. All personnel assigned to this contract shall limit their observation of holidays to those set forth above.
4.4. SECURITY REQUIREMENTS.
4.4.1. ACCESS. NO UNAUTHORIZED PERSONS ARE TO ACCOMPANY
WORKERS INTO THE BUILDING AT ANY TIME. THEFT, EMBEZZLEMENT, OR
DESTRUCTION OF U.S. GOVERNMENT PROPERTY IS A FEDERAL OFFENSE
AND WILL BE INVESTIGATED BY THE FEDERAL BUREAU OF
INVESTIGATION.
4.4.2. AGE REQUIRMENT. Due to the nature of Social Security requirements, all workers shall be at least eighteen (18) years of age and shall not have been convicted of a felony or theft misdemeanor. Final award of the contract is conditioned upon an acceptable background clearance of all employees having access to the building. An SF-
85 Questionnaire for Non-Sensitive Positions and fingerprint card will be completed for each employee of the contractor having access to the building.
4.4.3. GOVERNMENT ISSUED KEYS AND IDENTIFICATION CARDS.
(a) All Contractor personnel shall be required to wear identification cards, supplied by the
Government, while on the Government site. This requirement applies to personnel who are co-located at Government sites either on a full or part-time basis.
(b) It may become necessary for the Government to issue keys to the Contractor personnel.
(c) Prior to, or upon completion or termination of work required hereunder, the
Contractor shall return all such keys and identification cards to the CO or the duly authorized representative. In the event such keys or identification cards are not returned, the Contractor understands and agrees that the Government may, in addition to any other withholding provisions of the contact, withhold $50.00 for each key or identification card not returned. If the keys or identification cards are not returned within thirty (30) days from the date the withholding action was initiated, any amount so withheld shall be forfeited by the Contractor.
4.4.4. CONTRACTOR PERSONNEL SECURITY AND SUITABILITY
REQUIREMENTS. Performance of this contract requires contractor personnel to have a Federal government-issued personal identification card before being allowed unsupervised access to a DOI facility. The COR will be the sponsoring official, and will make the arrangements for personal identity verification and card issuance.
At least two (2) weeks before start of contract performance, the Contractor will identify all contractor and subcontractor personnel who will require physical access for performance of work under this contract. The Contractor must make their personnel available at the place and time specified by the COR in order to initiate screening and background investigations. The following forms, or their equivalent, will be used to initiate the credentialing process:
• OPM Standard Form 85 or 85P
• OF 306
• Fingerprint card (local procedures may require the fingerprinting to done at a police station; in this case, any charges are to be borne by the contractor),
• Release to Obtain Credit Information
• PIV card application (web-based)
Contractor employees are required to give, and to authorize others to give, full, frank, and truthful answers to relevant and material questions needed to reach a suitability determination. Refusal or failure to furnish or authorize provision of information may constitute grounds for denial or revocation of credentials. Government personnel may contact the contractor personnel being screened or investigated in person, by telephone or in writing, and the Contractor agrees to make them available for such contact.
Alternatively, if an individual has already been credentialed by another agency through
OPM, and that credential has not yet expired, further investigation may not be necessary.
Provide the COR with documentation that supports the individual’s status.
During performance of the contract, the Contractor will keep the COR apprised of changes in personnel to ensure that performance is not delayed by compliance with credentialing processes. Cards that have been lost, damaged, or stolen must be reported to the COR and Issuing Office within twenty-four (24) hours. Replacement will be at the contractor’s expense. If reissuance of expired credentials is needed, it will be coordinated through the COR.
At the end of contract performance, or when a contractor employee is no longer working under this contract, the Contractor will ensure that all identification cards are returned to the COR.
Before starting work under this contract, a National Agency Check (NAC) will be conducted to verify the identity of the individual applying for clearance. Upon successful completion of the NAC process, an identification card will be issued and access granted.
Simultaneously, a NAC with Inquiries (NACI) will be initiated to determine the individual’s suitability for the position. If the NACI adjudication is favorable, nothing more needs to be done. If the adjudication is unfavorable, the credentials will be revoked. In the event of a disagreement between the Contractor and the Government concerning the suitability of an individual to perform work under this contract, DOI shall have the right of final determination.
This requirement must be incorporated into any subcontracts that require subcontractor personnel to have regular and routine unsupervised access to a Federally-controlled facility for more than one-hundred eighty (180) calendar days or any unsupervised access to a Federally-controlled Level 3 or 4 information systems.
4.5. CONDUCT OF CONTRACTORS EMPLOYEES. The contractor shall prohibit employees from disturbing papers on desks, in drawers or cabinets, or from using telephones, or office equipment provided for government use.
Windows are to be closed, lights turned off when not in use; all "lost and found" items will be turned in to the CO or designated representative promptly. Hazardous, unsafe, or unhealthful conditions must be reported immediately. Items needing repair, such as leaky faucets, toilet blockages etc. should be reported immediately. The contractor is not authorized to adjust any thermostat in the facility.
4.6. PROCEDURES FOR AVOIDANCE OF ACTUAL OR APPARENT
PERSONAL SERVICES. In order to avoid actual or apparent personal services for contractor employees performing work at Government facilities the following procedures shall apply to this contract and subsequent task orders:
(i) Each contractor employee shall be provided, and shall wear in plain sight, an identification badge which clearly indicates they are a contractor employee.
(ii) A name plate or tag identifying the employee as an employee of the contractor shall be affixed in plain sight to the employee’s cubicle or office.
(iii) Contractor employees shall identify themselves as such in all correspondence, such as e-mails or faxes, originating from a Government provided source.
(iv) Contractor employees shall not attend Government all employee meetings or other inherently Governmental meetings unless specifically notified. All such notifications shall be coordinated through the CO and the on-site contract manager. If attendance at a recurring meeting is necessary for accomplishment of contractual requirements, a blanket notification may be executed.
(v) Government employees shall not participate in contractor employment interviews unless specifically approved by the CO and then only to the extent required to insure a skill match for the position being filled.
4.7. KEY PERSONNEL.
(a) The contractor shall assign to this contract the following key personnel who will have the full authority to act on behalf of the contractor during performance of efforts hereunder:
To be determined during negotiations (Individual names shall be included in the resultant contract as an attachment. Revisions in accordance with the provisions of this clause will be made by superseding letters to be included as a bilateral contract modification).
Supervision shall be the responsibility of the Contractor. Any Contract employee problems in performance, attendance, or other personal problems shall be referred to the
On-Site Supervisor to be referred on to the Contractor.
(b) During the first year of performance, the Contractor shall make no substitutions of those personnel unless the substitution is necessitated by illness, death, termination of employment, or otherwise approved by the CO. The Contractor shall notify the CO within fifteen (15) calendar days after the occurrence of any of these events and provide information required by paragraph (c) below. After the initial year, the Contractor shall submit the information required by paragraph (c) to the CO at least 15 calendar days prior to making any permanent substitution.
(c) Substitution of key personnel shall require the written approval of the CO. The
Contractor shall provide an explanation of the circumstances necessitating the proposed substitution, complete resumes for the proposed substitutes, and any additional information requested by the CO. Proposed substitutions should have comparable qualifications to those of the persons being replaced. The CO will notify the Contractor within 15 calendar days after receipt of all required information of the decision on substitutions. The key listing of personnel shall be modified by superseding letters as a bilateral contract modification as stated in (a) above.
(d) The Government reserves the right to refuse any contractor employees recommended or referred by the Contractor.
4.8. PROFESSIONAL STANDARDS AND ETHICS. All personnel furnished by the
Contractor shall be fully qualified for the services they are assigned. If any services performed by an individual do not conform to the contract requirements, or jeopardize successful performance of the contract, the CO may require the Contractor to immediately remove the individual from the contract at no additional cost to the
Government. The Contractor shall conduct only business covered by this contract during period paid for by the Government and shall not conduct any other business on
Government premises. Contractor employees assigned to this contract shall not solicit business that may be within the scope of this contract.
4.9. PAY AND BENEFITS.
(a) Regular hourly rates shall include fringe benefits, G&A, and overhead.
(b) Holiday pay and vacation time shall be in accordance with the Wage Determination included, and all revisions made to those Wage Determinations as they become effective.
(c) Contractor personnel are not entitled to administrative leave for such functions as picnics, blood drives, luncheons, dedications, etc.
(d) Contractor employees are not eligible to receive monetary awards from the
Government.
4.10. CERTIFIED EMPLOYEE TIME RECORDS. The Contractor shall maintain certified employee time records for the life of the contract. The records shall consist of a daily time log for each employee. The log shall be signed or initialed and certified as correct by the employee and the On-Site Supervisor. The time records for the period covered by an invoice shall be submitted with the invoice.
4.11. HOLD HARMLESS AND INDEMNIFICATION AGREEMENT. The
Contractor shall hold harmless and indemnify the Government against any and all liability, claims, and costs of whatsoever kind and nature for injury to or death of any person or persons and for loss or damage to any property occurring in connection with, in any way incident to, or arising out of the occupancy, use, service, operation or performance of work under the terms of the contract, resulting in whole or in part from the negligence acts or omissions of the Contractor, any subcontractor, or any employee, agent, or representative of the Contractor or subcontractor.
