STATEMENT OF WORK

FOR

CARPET REMOVAL AND INSTALLATION

1.0 Description of Service/Scope of Work. The contractor shall furnish all labor, tools, materials, permits, and equipment necessary to perform the work noted below in B922.

Work includes removing existing carpet and existing transitions, and installing new modular carpet squares, and transitions. (see attached drawing). Contractor is responsible for ensuring the actual/correct amount of carpeting required. Color choice is as follows:

CARPET MUST BE QUICK SHIP AND FINAL QUANTITIES MUST BE VERIFIED TO BE IN THE WAREHOUSE AT THE TIME

OF BIDDING.

The Contractor shall relocate furniture as required to complete the install. The Contractor is responsible for field measuring of carpet to insure exact amount of carpet required to complete this installation. Installation includes delivery, uncarting, set up and debris removal. Work must be done within normal business hours 0730‐

1600 M‐F unless coordinated with the user. The measurements given are an estimation. Offers shall include, but not be limited to: carpet and transitions.

The government requires the contractor to visit the site location and take their own measurements

(dimensions, quantities and square footages) prior to contract award. All dimensions, quantities and square footages included are approximations. The government is not responsible for incorrect measurements of the amount of carpet required. The extra carpet tile shall be given to the Facility Manager after the project is complete. The contractor shall haul away and dispose of removed carpet and transitions.

a. Contractor requirements for carpet replacement: Carpet to be purchased and installed shall be as noted above, lifetime commercial limited warranty, from Mandatory Sources. Time is of the essence.

b. Contractor is responsible for purchasing and installing transitions to be replaced in all applicable doorways where carpet tile is installed. The transitions shall match transitions or approved equal.

c. The existing floor surface shall be suitable for installation after the old carpet is removed. If the floor needs filling of holes or cracks, the contractor shall be responsible for making the floor carpet ready. Contractor shall furnish all cement, tape, thresholds and cap down moldings. Carpet to be installed in accordance with the manufacturer’s instructions. The contractor shall furnish all adhesive as recommended by carpet manufacture.

Contractor shall be responsible for preparation of floor surfaces as necessary to receive carpet. All surfaces to be free of voids and protrusions. Installed carpet shall be smooth, uniform, secure, and installed in a professional manner. Patterned goods shall be accurately matched. Fit cutouts such as doors, jambs, columns, steps, landings, and ducts neatly and securely.

d. Extra attention is to be observed while butting seams together. Contractor shall follow manufactures instructions on in matching the seams to prevent fraying and raveling and for unity in pattern. Contractor is to provide at least a five‐year warranty against fraying, raveling, separating, buckling and on his workmanship.

e. Phasing of work may be required to support the mission of the facility. Facility Manager will be the Point of Contact. Contract performance for each phase (if required) will start after the issuance of a NTP for that particular phase.

f. The Contractor is responsible for protection of existing facilities equipment from damage during removal of existing carpet and installation of new carpet. Contractor shall take into account the special treatment of hazardous waste, as well as the environmental implications related to building demolition management (i.e.

fluorescent lighting tubes, t‐stats, etc.). Every effort shall be made to recycle eligible materials, and those not suitable, shall be disposed of off base in accordance with all (SCDHEC) South Carolina Department of Health and

Environmental Control regulations.

g. Ensure that any demolition, utility cut and capping, and debris removal service performed at Shaw AFB, South Carolina in completed in such a manner that will restore a neat and professional appearance of base areas.

h. The facility/area shall be cleaned in its entirety, including exterior areas, before being returned the government.

i. During Performance, the contractor shall take care not to damage existing structures, etc. that will not be replaced as part of this contract. Contractor shall be responsible for the temporary support of all devices as required to perform work. Any demolition work that needs to be accomplished to complete the contract is the responsibility of the contractor. All material shall be installed per manufacturers’ recommendations. All items listed in the SOW are to be supplied and installed by the contractor unless otherwise noted.

j. The Contractor shall take all precautions to insure that his operations are conducted in a manner as to minimally interfere with the normal operations of the base and the safety and convenience of the base personnel.

The contractor shall coordinate utility outages with the Civil Engineering Squadron (CE) service call and the

Government representative.

1.2 Services Not Covered

The contractor shall notify equipment owner of any repairs required that are not listed in the scope of work and if any of the additional repairs would need to be completed before continuing with the maintenance outlined in the scope of work.

1.3 Quality Assurance

The government will periodically evaluate the contractor’s performance by appointing a Contracting Officer’s

Technical Representative(s) (COTR) to monitor performance to ensure services are received. The government representative will evaluate the contractor’s performance through intermittent on‐ site inspections of the contractor's quality control program and receipt of complaints from base personnel.

2.0. Government Furnished Property and Services: The Government shall provide, water, sewage, and electricity from existing sources.

3.0. General Information

3.1. All required work shall be coordinated through the Facility Manager, Sarah 803‐895‐1322, or Chuck Whittington @ 803‐895‐9584 to schedule time and date of installation.

3.2. The Contractor shall take no longer than 15 business days from beginning of installation to completion of installation.

3.3. Upon completion of the inspection or repair work within a facility, the contractor shall have the COR sign a copy of each Service Order/Work Request and a signed copy shall be given to the COR.

3.4. Safety. All work shall be in accordance with Federal, State and Base Safety regulations.

3.5. Hours of Operations. All non‐emergency work shall be accomplished during the normal duty hours of 0800 through 1630, Monday through Friday, excluding US Government holidays.

3.6. Shaw AFB recognizes the following holidays:

NOTE: Any of the above holidays falling on a Saturday will be observed on the proceeding Friday. Holidays falling on a Sunday will be observed on the following Monday.

4.0. Security Requirements for Unclassified Contracts

4.1. Listing of Employees. The contractor shall maintain a current listing of employees. The list shall include the employee's name, social security number and type of investigation if contract work involves unescorted entry to

Air Force restricted areas or other sensitive areas designated by the installation commander. The list shall be provided to the Program Manager and sponsoring agencies Security Manager. An updated listing shall be provided when an employee's status or information changes.

4.2. Pass and Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees.

4.2.1. Computer Network Access. N/A

4.3. Retrieving Identification Media. N/A

4.4. Traffic Laws. The contractor and its employees shall comply with base traffic regulations.

4.5. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor‐owned vehicle or privately‐ owned vehicle while on SHAW AFB.

4.6. For Official Use Only (FOUO). The contractor shall comply with DoD 5400‐7‐R, Chapter 4, DoD Freedom of Information Act (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

4.7. Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DOD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on‐base assignment.

4.8. Physical Security. The contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.
