Service Delivery Summary - Attachment 1
This document is a Service Delivery Summary table that outlines performance measures, acceptable quality levels, and surveillance methods for a neurosurgery contract with the Department of Veterans Affairs (SAVAHCS). The contract involves the provision of Intraoperative Neuromonitoring Services (IONMS) by licensed contractors in Arizona. Key service requirements include: pre-approval authorization for surgical procedures with 100% quality acceptance; coordination with the facility for scheduled procedures within 24 hours of referral; reporting of complications within 24 hours of discovery to the Surgical Care Line; completion of all administrative tasks and medical documentation within 24 hours of procedures; and maintenance of a Quality Assurance program with quarterly tracked and trended data submission. All personnel must be licensed in Arizona and subject to Department of Veterans Affairs review.
The surveillance methodology relies on observation and random auditing by the Contracting Officer Representative (COR) and Neurosurgery staff using Quality Assurance Survey/Checklists that assess read/interpret accuracy, technical problems, and equipment performance. Additional requirements include monthly compliance audits for patient confidentiality in accordance with the Privacy Act, Title 5 U.S.C. 552a, Title 38 U.S.C. 5701, 5705, 7362, and HIPAA under Public Law 104-191. All performance measures maintain a 100% acceptable quality level, and personnel qualifications must be reviewed at contract award and whenever new contracted employees are assigned.
