STATEMENT OF WORK
FOR
Basic Military Training Dormitory Mattress Delivery, Installation and Disposal
PROJECT LOCATION: The physical address for building 6586 is 1551 Connally St JBSA-Lackland Texas 78236.
PROJECT DESCRIPTION:
The contractor shall furnish all labor, supervision, materials, supplies, tools, equipment, and expertise necessary to perform the installation of all mattresses in each dormitory of building 6586. The contractor shall work closely with the designated Contracting Officer Representative (COR) to remove and properly dispose of all trash, plastic and old mattresses, being replaced in building 6586. The contractor shall ensure there is a way to remove the old disposable mattresses, plastic from mattresses and trash before and during the delivery of the new mattresses. CORs will be TSgt Patrick O’Neil, patrick.oneil.1@us.af.mil and Joshua Reynolds, joshua.reynolds.5@us.af.mil, commercial telephone (210) 671 9584. Procure 1,250 Dormitory twin mattresses, for the Basic Military Training (BMT) Squadron 321nd TRS, 1551 Connally St. (Building 6586) at Joint Base San Antonio (JBSA), Lackland, Texas with delivery.
Building 6586 [Basic Military Training (BMT) Squadron 321st TRS – Requesting 1,250 new mattresses be delivered, remove plastic and installed into Dormitory Rooms.

The building does have four stairwells and an elevator that provides access to the upper floors. 1,252 old mattresses and plastic shall be removed from the dormitory bays and disposed of. This will be a one to one swap. Contractor shall make arrangements for proper disposal of old mattresses, mattress plastic, and all trash daily. They are not to be disposed of anywhere on the military installation. Roll off dumpster are not to be placed or used on the installation. Old Mattresses, mattress plastic, and all trash must be disposed of using off base disposal resources of the contractor’s choosing and transported using contractor provided trucks.
Point of Contact: The project may be completed delivered and replaced within one week of start of delivery to ensure personnel living within the facility have a new mattress replacement in place as needed. All pertinent communications should be written or confirmed in writing. The building Facility Managers will act as points of contact throughout the process. The facility managers are TSgt Patrick O’Neil, patrick.oneil.1@us.af.mil , Joshua Reynolds, joshua.reynolds.5@us.af.mil, at commercial telephone (210) 671-9584 and Terrell Thames at terrell.thames.2@us.af.mil. (210) 671-2078.
PROJECT MANAGEMENT
The Contractor shall identify and provide a single point of contact (POC) to provide overall management and supervision during the term of this project. The single POC shall serve as liaison for the customer representatives to ensure the delivery of mattresses. It is necessary to ensure mattresses are successfully delivered and replaced in a timely manner to ensure personnel living within the facility have a new mattress replacement in place as needed. The contractor must do a one for one swap of the old mattresses removed for new mattresses delivered each day.
Location of Delivery and Installation:
This project requires delivery and installation of 1,250 mattresses to building 6586.
Note: Building 6586 is a four (4)-story dormitory located on JBSA-Lackland with physical address: 1551 Connolly St., JBSA-Lackland Texas 78236. The building has four (4) stairwells and a single elevator.
All materials and accessories shall be installed in accordance with industry standards and manufacturer’s recommendations. The contractor will coordinate all delivery times with the customer.
HOURS OF OPERATION: The delivery and installation shall be performed during normal duty hours (7:30 A M. to 3:30 P. M., Monday thru Friday).
BASE ACCESS: The contractor shall contact the following primary POCs:
Mr. Terrell Thames (210) 671-2078/2103, Chad Cowdery (210) 671-2078, (210) 671-2103, Mr. David Cross and or Mr. David Cranek (210) 671-9393 at least 72 hours prior to delivery to gain authorized access onto the military installation. The contractor must access the installation using the Growdon Gate.
ACCESSORIES AND INCIDENTALS: The Contractor shall be responsible for any damage to the building which occurs during delivery and installation. It is incumbent for the contractor to identify any existing damage to the building by means of a Pre-Installation and Delivery Walk-Thru, and submissions of checklists to the designated agency representative on site.
QUALITY ASSURANCE: The government shall inspect all work performed by the contractor. The inspection shall be completed by either of TSgt Joshua Reynolds, TSgt Patrick O’Neil and or Mr. Terrell Thames reported to 802CES.
Work site: The contractor shall clean up and dispose of waste materials or rubbish in accordance with this paragraph and all applicable federal, state & local regulations, standards, codes & laws. The contractor shall remove and dispose all old mattresses being replaced daily. Do not dispose of or discard recyclables, packing materials, or trash in any Government recycling, trash containers, or dumpsters located on the installation.
Protection of New Building and Property Contractors will provide and utilize the proper measures (protective runners, pads, etc.) to protect the building (stairs, stairwells, walls, floors, carpeted areas, etc.) from damage during the installation and removal. The protective materials shall not be bolted, glued or stapled to floors, carpeted areas, walls, ceilings, or doors but instead shall either be free standing or braced so as not to cause any damage. The contractor shall establish and maintain this protection and maintain it throughout the full term of the purchase order. Any damage caused by the contractor shall be the responsibility of the contractor to repair or replace.
After the installation of each phase, the contractor and government representative shall jointly conduct a walk-through and annotate any new damage at the new location. Any damage caused by the contractor will be the responsibility of the contractor to repair or replace at no additional cost to the Government.
ENVIRONMENTAL REQUIRMENTS: The contractor shall perform all work in accordance with federal, state, and local environmental regulations.
