Document Title Pest Control Services for Lower Colorado Basin Office Buildings
SERVICES REQUIRED
A licensed pest-control Contractor shall provide all labor, equipment, and materials unless noted otherwise to perform pest-control service for all Lower Colorado Basin Regional Office Buildings for the Bureau of Reclamation, Boulder City, NV, as described later in this performance work statement. Pest-control service shall include control of common vertebrates and invertebrates normally found in the local area including but not limited to ants, roaches, silverfish, rats, mice, spiders, scorpions, and other insects, etc. Special pest-control services for less commonly occurring insects, reptiles, rodents and animals, and invasive or noxious plants are excluded from this contract but may be required under a separate contract or by modification at a later date, if needed.
INTEGRATED PEST MANAGEMENT
Integrated Pest Management (IPM) is a concept that establishes a sustainable approach to managing pests by combining biological, cultural, physical, and chemical tools in a way that minimizes economic, health, and environmental risks. Decisions concerning whether or not pesticides should be applied in a given situation shall be based on a review of all available options. Efforts shall be made to avoid the use of pesticides by adequate pest-proofing of facilities, good sanitation practices, selection of pest-resistant plant materials, and appropriate horticultural practices. When it is determined that a pesticide must be used in order to meet pest-management objectives, the least-hazardous material, adequate for the job, shall be utilized.
1.0 SERVICES
CONTRACTOR PLAN OF OPERATION
A. The Contractor shall submit to the Contracting Officer (CO) or Contracting Officer’s
Representative (COR) an Operations Plan at least thirty (30) working days prior to the starting date of the contract. If aspects of the Plan are incomplete or disapproved by the CO/COR, the Contractor shall have two (2) working days to submit revisions. The Plan shall consist of three (3) parts, as follows:
1. Pesticide Labels, Safety Data Sheets (SDS), Pesticide Use Proposal, and Hazardous
Materials Authorization Form: The Contractor shall provide current Labels and SDS for all pesticides that shall potentially be used in the pest-control program.

Additionally, the Contractor shall complete and provide the Pesticide Use Proposal from and Bureau of Reclamation Lower Colorado Region Hazardous Materials Authorization Form (attached).
2. Service Schedule(s): The Contractor shall provide a schedule of routine pest-control inspections for each building serviced under this contract, including frequencies of inspections, areas at each facility to be given special attention (e.g. food storage, preparation, and serving areas, washrooms, custodial closets, mechanical rooms, entryways) and specific day(s) of the week on which inspections shall be performed.
Regular pest-control inspections and service shall be performed a minimum of twelve (12) times per year. The rate of regular inspections shall be at an interval of no less than twenty-one (21) days and no more than twenty-eight (28) days.
Work may be performed Monday - Friday, from 6:00 a.m. to 4:30 p.m., recognizing that some chemical applications may need to be applied after regular working hours for the least amount of impact to staff. The schedule for each visit, whether a regularly scheduled visit or an additional spraying, shall be established jointly by the COR and Contractor not less than one (1) week prior to spraying, to ensure all customers are properly notified and staff and security/access scheduling at Facility Management Group and associated buildings is timely and appropriate.
3. Commercial Pesticide Applicator Licenses and Certificates: The Contractor shall provide a copy of the State-issued Commercial Pesticide Applicator License for every Contractor employee performing on-site pest-control service under this contract, and a copy of the State-issued Commercial Pesticide Applicator Certificate for every pest-management professional (PMP) performing on-site pest-control service.
B. Throughout the contract period, the Contractor shall provide recommendations for future mechanical, environmental, and chemical control of pests common at each building serviced.
C. The Contractor shall receive the approval of the CO or COR prior to implementing any subsequent changes to the approved Plan, including additional or replacement pest-control products.

2.0 RECORDKEEPING
The Contractor shall be responsible for maintaining a logbook or file for each building specified in this contract. These records shall be kept on site and maintained on each visit by the PMP performing pest-control service. Each logbook or file shall contain at least the following items:
A. Operations Plan: A copy of the Contractor’s approved Operations Plan, including pesticide labels and SDS for all pesticides that shall be potentially used in the building, the services schedule for routine pest-control inspections, and copies of the relevant Commercial Pesticide Applicator Licenses and Certificates.
B. Building Occupant Log Form: These forms will be used to advise the Contractor of routine service requests and pest sightings by building occupants.
C. Contractor’s Report Forms: Customer copies of Contractor’s signed and dated Service Report Form, documenting all information on services provided, including pesticide applications required by State and local statutes. This form must also indicate any recommendations made by the Contractor for additional action advisable to be taken by the customer, e.g. structural or plumbing repairs required to limit pest access to the building or to food and water resources; improvements in sanitation, etc. A copy of this form must also be provided to the CO/COR within one (1) week of the service.
D. Contractor Products and Devices: All bait stations, snap traps, glue boards, and other devices left behind by the Contractor, shall be dated, numbered, and listed on the Contractor Report Form and checked on each subsequent visit until removed. All such devices shall be removed when full, dirty, and no longer effective, or no longer needed.
3.0 MANNER AND TIME TO CONDUCT SERVICE
A. Time Frame of Service Visits: Frequent and clear communication between the Contractor and COR is critical for a successful outcome. Routine pest-control services that do not adversely affect staff health or productivity shall be performed during the regular building hours of operation. For the intent of this PWS, regular building hours of operation is Monday through Friday, 0800 – 1600 hours. When necessary to perform work outside of the regularly scheduled service time set forth in the Contractor Operations Plan, the Contractor shall notify the CO/COR at least one (1) day in advance.

B. Re-Entry Notifications: If the Government determines the application of a chemical pesticide is warranted (based on recommendation by the Contractor or by observations indicating the application of non-chemical means are not effective), electronic notifications will be made to affected employees at least 72 hours in advance. This electronic notification will describe the date, time, and specific locations where the application will occur. Potential adverse health effects (based on information contained within the SDS) will also be included in the notification. In the event an emergency chemical application is required, this 72-hour notice will not be required; however, the Government will make necessary electronic notifications as soon as possible. If chemical pesticide application occurs within restroom or break areas, the contractor will post a notice at the affected area, which will include possible adverse health effects, based on the SDS. This post-application notification will also include the duration for the notice to be in effect.
C. Safety & Health: All pesticide storage, transportation, and application shall be in strict accordance with all applicable Federal, State, and local environmental, safety, and health requirements, laws, and regulations. Where there is a conflict between applicable regulations, the most stringent applies. Under no circumstances shall the Contractor store any pesticides within the Lower Colorado Regional Office buildings or grounds.
D. Special Entrance: Certain areas within some buildings may require special instructions for persons entering them. Any restrictions associated with these special areas will be explained by the CO/COR. The Contractor shall adhere to these restrictions and incorporate them into the Contractor Operations Plan.
E. Uniforms: All Contractor employees working in or around the buildings specified in this contract shall wear distinctive uniforms identifying the name of the Contractor employee and the Contractor.
F. Vehicles: Vehicles used by the Contractor shall be identified in accordance with State and local regulations. Vehicles used by the Contractor shall be registered and insured in accordance with Nevada State law.
4.0 SPECIAL REQUESTS AND EMERGENCY SERVICE
On occasion, the CO/COR may request that the Contractor perform corrective, special, or emergency service(s) that are beyond routine service requests, such as removal of a stinging

Document Title Pest Control Services for Lower Colorado insect nest. The Contractor shall respond to these exceptional circumstances and complete the necessary work within twenty-four (24) hours of receipt of the request.
5.0 INSECT CONTROL
A. Emphasis on Non-Pesticide Methods: Non-pesticide methods of control shall be used wherever possible, for example:
1. Portable vacuums, rather than pesticide applications, shall be the standard method for initial clean-outs of cockroach infestations, for swarming (winged) ants and termites, and for control of spiders/webs.
2. Trapping devices, rather than pesticide applications, shall be the standard method for indoor fly control.
B. Application of Insecticides to Cracks and Crevices: As a general rule, all insecticides shall be applied as “crack and crevice” treatments only, defined in this contract as treatments in which the formulated insecticide is not visible to a bystander or accessible to children or after the application process.
C. Application of Insecticides to Exposed Surfaces or as Space Sprays: Application of insecticides to exposed surfaces or as space sprays (fogging) shall be restricted to exceptional circumstances where no other reasonable alternative measures are practical.
The Contractor shall obtain approval of the CO/COR prior to any application of insecticide to an exposed surface or any space spray treatment. No surface application or space spray shall be made while staff or visitors are present. The Contractor shall take all necessary precautions to ensure staff and visitor safety, and all necessary steps to ensure the containment of the pesticide to the application site.
D. Insecticide Bait Formulations: Bait formulations shall be the standard pesticide technology for cockroach and ant control, with alternate formulations restricted to unique situations where baits are not practical.
E. Monitoring: Sticky traps shall be used to guide and evaluate indoor insect control efforts wherever necessary.

The Contractor shall check in with the Facility Maintenance Group (500 Date Street, Building 1200, Boulder City, NV 89005) upon arrival and prior to performing work.
6.0 SAFETY AND SECURITY
A. General: Contractor employees shall operate equipment in compliance with all Federal, State, and local safety codes, and in accordance with the manufacturer’s guidelines and manuals.
B. Safety and Health: The Contractor shall not require any Contractor employee in the performance of this contract to work under conditions which are unsanitary, hazardous, or dangerous to the Contractor employee’s health or safety, as determined under safety and health standards promulgated by the Secretary of Labor under Section 107 of the Contract Work Hours and Safety Standards Act (40 U.S.C. 327 et seq.), as amended, and Reclamation Safety and Health Standards (RSHS) (2002 Edition), including the ERRATA dated 2005, published by the Bureau of Reclamation. The Contractor may obtain the RSHS manual from the Bureau of Reclamation’s website at: http://www.usbr.gov/ssle/safety/RSHS/rshs.html
The Contractor shall comply with all applicable safety and occupational health requirements set forth in 29 CFR 1910, OSHA’s General Industry Standards and with the (RSHS) (2014 Edition), including the ERRATA dated 2014, published by the Bureau of Reclamation. In the case of a conflict between 29 CFR 1910, OSHA’s General Industry Standards and the (RSHS) (2014 Edition), including the ERRATA dated 2014, the more stringent requirements apply. If the Contractor fails or refuses to promptly comply with safety requirements, the CO may issue an order stopping all or part of the work until satisfactory corrective action has been taken.
The Bureau of Reclamation Regional Safety Office will review the Contractor’s “Safety Program” for compliance with the RSHS. Job Hazardous Analyses (JHA) submitted for approval must be site-specific and follow guidelines set forth in the RSHS. Both Safety Plan and JHAs must be submitted for approval at least two (2) weeks prior to the first spraying.
C. Security: In accordance with Homeland Security Presidential Directive-12 (HSPD-12), Contractor personnel maybe required to undergo the Government-issued Personal Identity Verification (PIV) credentialing process before being allowed access to work at the site. The cost of suitability and PIV-related background investigations will be borne by the Government.

Following the submittal to the COR of the investigation package for each employee, the employee will receive an invitation to provide additional information required for their background investigation through the on-line e-QIP process. The background investigation will be conducted by the Lower Colorado Region Security Office. Contractor employees may not be assigned to a Reclamation facility until advised by the COR that a preliminary favorable suitability determination by the Government has been completed.

The Contractor shall be responsible for ensuring that its employees comply with all established security rules and regulations. Contract personnel shall not be permitted to enter established or designated restricted areas unless so authorized by the Government.
Entry shall be in accordance with and subject to the security regulations established in the area. It shall be the Contractor’s responsibility to ensure that its personnel do not enter these areas unless authorized.

The Contractor shall provide pest-control services for the above-listed buildings, which shall include:
Interior: All offices, hallways, elevator entrances, vending machine areas, kitchens, conference rooms, restrooms, stairwells, mechanical rooms, and storage rooms.
Exterior: Building exterior perimeter, breezeways, cabana areas, dumpster staging areas, and around all exterior doors.
7.0 INSPECTION AND ACCEPTANCE OF PERFORMANCE
All inspections will be completed at the time services are performed and will consist of direct observation/surveillance of work performed.
The services performed by the contract are of such a nature that defective or incomplete performance, disclosed by Government inspection, requires immediate completion or re-performance. This shall be accomplished prior to any payment for such services and shall be scheduled and accomplished within 48 hours of the originally scheduled date of performance.
Full and complete coverage of all scheduled areas is necessary for effective pest control.
Skipped or missed areas are not acceptable.
At the sole discretion of the Government, the Government may determine that specific locations shall not be treated due to other conflicting work in such areas or for other reasons for which the Government deems work under this contract to be inappropriate at the scheduled time. The CO/COR will notify the Contractor if this is the case.
When the Government elects to disallow treatment, the CO will then provide the Contractor with a determination regarding pro-rata payment of services performed and/or later scheduling for such areas.
When such rescheduling is required, the Contractor shall perform the rescheduled work under inspection/surveillance as outlined above; however, all work performed as regularly scheduled and not subject to delay rescheduling will be considered complete and not subject to further inspection/re-inspection.

8.0 COORDINATION
A. Scheduling and coordination of services under this contract shall be accomplished through the CO/COR.
B. Safety Program and related safety matters (SDS) shall be submitted through the CO/COR for review and acceptance by the Regional Safety Officer and other appropriate Reclamation staff.
C. Any other work contemplated by the CO/COR not described herein, if requested, shall be referred to the CO for approval to proceed, or for separate purchase under contract modification/purchase order, prior to proceeding. Failure to obtain approval prior to completing additional work may result in nonpayment for services.
